摘要
目的:探讨阿替普酶联合替罗非班治疗急性心肌梗死(AMI)的效果及对患者凝血功能、心肌功能的影响。方法:选择2020年9月—2022年9月在盘锦市中心医院接受治疗的AMI患者74例,按随机数字表法分为两组。阿替普酶组(n=37)给予阿替普酶治疗,联合组给予阿替普酶+替罗非班治疗。比较两组治疗效果、凝血功能[(凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT))]、心肌功能[心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)]及不良事件发生情况。结果:联合组的总有效率(94.59%)高于阿替普酶组(78.38%)(P<0.05);联合组治疗后的PT、TT、APTT均明显长于阿替普酶组(P<0.05);联合组治疗后的CK-MB、cTnI水平均明显低于阿替普酶组(P<0.05);联合组的不良事件发生率(10.81%)明显低于阿替普酶组(29.73%)(P<0.05)。结论:阿替普酶联合替罗非班治疗AMI效果确切,可减少不良事件的发生,对AMI患者的凝血功能、心肌功能具有明显改善作用。
Objective:To investigate the effect of Alteplase combined with Tirofiban in the treatment of acute myocardial infarction(AMI)and its influence on coagulation function and myocardial function of patients.Method:A total of 74 patients with AMI who received treatment in Panjin Central Hospital from September 2020 to September 2022 were selected and divided into two groups according to random number table method.Alteplase group(n=37)was treated with Alteplase,and combination group was treated with Alteplase+Tirofiban.The therapeutic effect,coagulation function[prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT)],myocardial function[cardiac troponin I(cTnI),creatine kinase-MB(CK-MB)]and the occurrence of adverse events were compared between the two groups.Result:The total effective rate of combination group(94.59%)was higher than that of Alteplase group(78.38%)(P<0.05).After treatment,the PT,TT and APTT in combination group were significantly longer than those in Alteplase group(P<0.05).After treatment,the levels of CK-MB and cTnI in combination group were significantly lower than those in Alteplase group(P<0.05).The incidence of adverse events in combination group(10.81%)was significantly lower than that in Alteplase group(29.73%)(P<0.05).Conclusion:Alteplase combined with Tirofiban is effective in the treatment of AMI,which can reduce the occurrence of adverse events and significantly improve the coagulation function and myocardial function of AMI patients.
