结直肠癌术后肝转移FOLFIRI化疗中全身静脉与肝动脉灌注方式的临床对照研究

Clinical Comparative Study of Systemic Venous and Hepatic Artery Perfusion during FOLFIRI Chemotherapy for Liver Metastasis after Colorectal Cancer Surgery

目的:分析结直肠癌术后肝转移FOLFIRI化疗中应用全身静脉与肝动脉灌注方式的不同效果。方法:选择2018年7月—2019年8月赣南医学院第一附属医院收治的70例结直肠癌术后肝转移患者作为研究对象,随机将其分为对照组(n=35)和观察组(n=35)。对照组患者采用全身静脉方式治疗,观察组患者采用肝动脉灌注方式治疗。比较两组近期疗效、不良反应和生存率。结果:观察组疾病控制率和客观缓解率比较,差异均无统计学意义(P>0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05);两组1年生存率对比,差异无统计学意义(P>0.05),观察组2年生存率和3年生存率均高于对照组,差异均有统计学意义(P<0.05)。结论:结直肠癌术后肝转移FOLFIRI化疗中采用肝动脉灌注的方式,治疗效果显著,安全性高。

Objective:To analyze the different effects of systemic venous and hepatic artery perfusion of FOLFIRI in liver metastasis after colorectal cancer surgery.Method:A total of 70 patients with liver metastasis after colorectal cancer surgery admitted to First Affiliated Hospital of Gannan Medical University from July 2018 to August 2019 were selected as the study objects,and randomly divided into control group(n=35)and observation group(n=35).Patients in the control group were treated with systemic venous,and patients in the observation group were treated with hepatic artery perfusion.The short-term effect,adverse reactions and survival rate were compared between the two groups.Result:There were no significant differences between disease control rate and objective response rate between the two groups(P>0.05).The incidence of adverse reactions in observation group was lower than that in control group,the difference was statistically significant(P<0.05).There was no statistical significance in the 1-year survival rate between the two groups(P>0.05).The 2-year and 3-year survival rates in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Hepatic artery perfusion is used in FOLFIRI chemotherapy for liver metastasis after colorectal cancer surgery,which has significant therapeutic effect and high safety.
