旋覆代赭汤加减治疗脑卒中后吞咽功能障碍的临床观察

Clinical observation of modified Xuanfu Daizhe Decoction in the treatment on post-stroke dysphagia

目的 探讨旋覆代赭汤加减治疗脑卒中后吞咽功能障碍患者的临床效果。方法 收集榆林市中医医院脑病科2015年1月至2020年3月就诊的中风后吞咽功能障碍患者100例,随机分为研究组与对照组,各50例,研究组使用旋覆代赭汤加减联合常规西医治疗,对照组仅使用常规西医治疗。比较两组治疗前后洼田饮水试验(water swallowingtest,WST)、藤岛一郎吞咽障碍评分、吞咽障碍特异性生活质量量表评分(swallowqualityoflife questionnaire, SWAL-QOL)。结果 治疗后研究组临床总有效率为94.00%,对照组总有效率为78.00%,两组患者WST评分降低,藤岛一郎吞咽障碍评分、SWAL-QOL评分均较治疗前升高,且研究组优于对照组,差异均有统计学意义(P<0.05)。结论 旋覆代赭汤加减治疗能有效地减轻中风后吞咽功能障碍患者的症状,改善患者生活质量。

Objective To evaluate the clinical effect and safety of modified Xuanfu Daizhe Decoction in the treatment on post-stroke dysphagia.Methods 100 patients with post-stroke dysphagia admitted to Yulin Traditional Chinese Medicine Hospital from January 2015 to March 2020 were randomly divided into the study group and the control group with 50 cases in each group.The patients in both groups were given routine treatment of Western medicine,and the study group was given Xuanfu Daizhe Decoction on the basis of the control group.The score of the water swallowing test(WST),Fujishima Ichiro swallowing disorder,and swallow quality of life questionnaire(SWAL-QOL)before and after treatment between the two groups were observed.Results The clinically total effective rate of the study group was 94%after treatment,which was higher than that of the control group with total effective rate of 82%.The scores of WST in the two groups were decreased,and the score of Fujishima Ichiro swallowing disorder,and swallow quality of life questionnaire of patients in the two groups increased after treatment,and the above score in the study group was significantly higher than that in the control group(P<0.05).Conclusion The modified Xuanfu Daizhe Decoction can effectively alleviate the symptoms of patients with post-stroke dysphagia,improving patient the quality of life.
