2017-2022年山东省汶上县居民新发恶性肿瘤流行病学特征分析

Analysis on epidemiological characteristics of denovo malignancy among residents in Wenshang county,Shandong province from 2017 to 2022

目的探讨2017-2022年山东省汶上县居民新发恶性肿瘤发生情况,为恶性肿瘤防治工作提供依据。方法在山东省慢性病(肿瘤)监测信息系统中,收集2017-2022年山东省汶上县13处乡镇和2个街道长期居住的14311例恶性肿瘤新发病例资料,对发病资料进行回顾性分析。结果描述性分析表明,2017-2022年山东省汶上县新发恶性肿瘤报告发病率逐年上升。年均粗发病率为290.72/10万,标化率为153.67/10万。其中男性新发病例(7582例)报告发病率为294.76/10万,标化率为156.38/10万;女性新发病例(6729例)报告发病率为286.30/10万,标化率为152.96/10万(χ^(2)=27229.69,P<0.005)。不同年龄组恶性肿瘤报告发病率随年龄增长呈上升趋势,15~19岁发病率最低(8.69/10万),≥85岁发病率最高(3647.59/10万)。不同乡镇恶性肿瘤报告发病率比较差异有统计学意义(χ^(2)=945.98,P<0.005),汶上街道最高(564.96/10万),中都街道最低(187.72/10万)。前5位的恶性肿瘤依次为肺癌(发病率55.01/10万,标化率26.24/10万)、食管癌(发病率50.40/10万,标化率22.50/10万)、胃癌(发病率27.67/10万,标化率13.08/10万)、乳腺癌(发病率25.31/10万,标化率15.25/10万)和肝癌(发病率19.46/10万,标化率10.10/10万),占报告发病总数的61.18%。结论山东省汶上县居民恶性肿瘤处于高发状态,以男性、中老年人群、肺癌、食管癌和胃癌为主。

Objective To explore the incidence of denovo malignancy among residents in Wenshang county,Shandong province from 2017 to 2022,so as to provide a basis for the prevention and treatment of malignant tumors.Methods A total of 14311 new cases of malignant tumors in 13 towns and 2 streets of Wenshang county from 2017 to 2022 were collected from the Chronic Disease(Tumor)Surveillance Information System of Shandong Province,and the incidence data of 14311 cases of malignant tumors were analyzed retrospectively.Results From 2017 to 2022,the reported incidence of denovo malignancy in Wenshang county,Shandong province increased year by year,with an average annual crude incidence of 290.72/10^(5) and a standardized rate of 153.67/10^(5).Among them,the reported incidence of male new cases(7582 cases)was 294.76/10^(5),and the standardized rate was 156.38/10^(5);The reported incidence of female new cases(6729 cases)was 286.30/10^(5),and the standardized rate was 152.96/10^(5)(χ^(2)=27229.69,P<0.005);The reported incidence of malignant tumor in different age groups increased with age,those aged 15-19 had the lowest incidence(8.69/10^(5)),and those aged 85 and above had the highest incidence(3647.59/10^(5));There was statistical significance in the reported incidence of malignant tumor among different towns(χ^(2)=945.98,P<0.005).The highest incidence was in Wenshang street(564.96/10^(5)),and the lowest was in Zhongdu street(187.72/10^(5)).The top 5 cases with malignant tumors were lung cancer(incidence 55.01/10^(5),standardized rate 26.24/10^(5)),esophageal cancer(incidence 50.40/10^(5),standardized rate 22.50/10^(5)),stomach cancer(incidence 27.67/10^(5),standardized rate 13.08/10^(5)),breast cancer(incidence 25.31/10^(5),standardized rate 15.25/10^(5))and liver cancer(incidence 19.46/10^(5),standardized rate 10.10/10^(5)),accounting for 61.18%of the total reported cases.Conclusion The incidence of malignant tumors is high in Wenshang county,Shandong province,dominated by males,middle-aged and elderly populations,and lung,esophageal and stomach cancers.
