大环内酯类抗生素辅助支气管镜治疗儿童难治性肺炎的疗效观察

摘要 目的探究大环内酯类抗生素辅助支气管镜治疗儿童难治性肺炎的效果。方法选取喀什地区第一人民医院于2021年1月-2022年11月收治的96例难治性肺炎患儿作为研究对象,按照随机数字表法分为治疗组(48例)与对照组(48例),对照组使用大环内酯类抗生素治疗,治疗组使用大环内酯类抗生素辅助支气管镜治疗,比较两组治疗效果、临床症状好转时间及实验室指标水平[血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超氧化物歧化酶(SOD)、丙二醛(MDA)]。结果治疗组的治疗有效率高于对照组(P<0.05);治疗组患儿体温恢复正常时间、咳嗽消失时间及住院时间短于对照组患儿(P<0.05);治疗前,两组患儿IL-6、TNF-α、MDA、SOD等指标水平差异不大(P>0.05),治疗后,治疗组患儿IL-6、TNF-α、MDA等指标水平低于对照组,SOD水平高于对照组(P<0.05)。结论予以难治性肺炎患儿使用大环内酯类抗生素辅助支气管镜治疗可获得良好效果,快速改善各项临床症状,降低炎症因子水平,减轻氧化损伤,值得临床推广。 Objective To explore the effect of macrolide antibiotics assisted bronchoscopy in the treatment of refractory pneumonia in children.Method A total of 96 children with refractory pneumonia admitted to our hospital from January 2021 to November 2022 were selected and divided into treatment group(48 cases)and control group(48 cases)according to random number table method.The control group was treated with macrolide antibiotics,and the treatment group was treated with macrolide antibiotics assisted bronchoscopy.The therapeutic effect,clinical symptom improvement time and laboratory index levels(serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),superoxide dismutase(SOD),malondialdehyde(MDA))were compared between the two groups.Results The effective rate of treatment group was higher than that of control group(P<0.05).The temperature recovery time,cough disappearance time and hospitalization time in treatment group were shorter than those in control group(P<0.05).Before treatment,the levels of IL-6,TNF-αMDA and SOD were not significantly different between the two groups(P>0.05).After treatment,the levels of IL-6,TNF-α.and MDA in the treatment group were lower than those in the control group,while the levels of SOD in the treatment group were higher than those in the control group(P<0.05).Conc lusion Bronchoscopy assisted by macrolide antibiotics in children with refractory pneumonia can achieve good results,rapidly improve various clinical symptoms,reduce the level of inflammatory factors and reduce oxidative damage,which is worthy of clinical promotion.
