摘要
目的探讨早期舒适度护理在呼吸衰竭低氧血症患者经鼻高流量湿化氧疗(HHFNC)中的应用。方法自2021年9月-2023年5月期间选取呼吸衰竭低氧血症患者84例,应用随机数字表法将患者分为观察组及对照组,每组各42例,两组患者均行HHFNC治疗,对照组HHFNC治疗期间行基础性护理,观察组HHFNC治疗期间行早期舒适度护理,比较两组舒适度评分、治疗依从性评分、氧合指标、躁动发生率、意外拔管率、ICU停留时间及患者满意度评分。结果两组治疗结束时氧合指标改善效果明显优于治疗前(P<0.05)。观察组干预后舒适度评分、治疗依从性评分及患者满意度评分明显高于对照组(P<0.05)。观察组躁动发生率、意外拔管率低于对照组(P<0.05),观察组ICU停留时间短于对照组(P<0.05)。结论早期舒适度护理可提高呼吸衰竭低氧血症患者HHFNC治疗期间舒适度,降低患者HHFNC治疗期间躁动及意外拔管发生率,缩短患者治疗时间,提高患者治疗满意度。
Objective To explore the application of early comfort assessment and management plan in patients with respiratory failure hypoxemia and high flow humidification oxygen therapy(HHFNC).Methods 84 cases of patients with respiratory failure and hypoxemia were selected from September 2021 to May 2023.The patients were divided into observation group(n=42)and control group(n=42)by random number table.Both groups were treated with HHFNC.The control group were received with basic care during HHFNC treatment.The observation group were received with early comfort assessment during HHFNC treatment and implemented targeted nursing interventions based on the assessment.The comfort scores,treatment compliance scores,oxygenation indicators,and agitation.Rate,accidental extubation rate,ICU stay time and patient satisfaction score of two groups were compared.Results The oxygenation index improvement effect of two groups at the end of treatment were significantly better than before treatment(P<0.05).The comfort score,treatment compliance score and patient satisfaction score of observation group after intervention were significantly higher than control group(P<0.05).The incidence of restlessness and accidental extubation of observation group were lower than control group(P<0.05),and the ICU stay time of observation group were shorter than control group(P<0.05).Conc lusion Early comfort assessment and management programs can improve the comfort of patients with respiratory failure hypoxemia during HHFNC treatment,reduce the incidence of restlessness and accidental extubation during HHFNC treatment,shorten the treatment time of patients,and improve patient satisfaction with treatment.
