隧道式PICC置入术在临床中的应用

摘要 目的 探讨隧道式中心静脉导管(peripheraally inserted central cathcter,PICC)置入术在临床中的应用效果。方法 选取萍乡市第二人民医院2020年1月—2022年5月收治的60例恶性肿瘤患者(需要肠外营养支持或长期静脉化疗),随机分为研究组(n=30)和对照组(n=30)。对照组患者采用传统PICC导管治疗,研究组患者采用隧道式PICC置入术治疗。随访2组患者穿刺成功及并发症情况,综合评价临床疗效,比较2种不同置管方法的差异。结果 研究组一次穿刺成功率(96.67%)高于对照组(80.00%)(P<0.05)。研究组48 h内渗血发生率(3.33%)、机械性静脉炎发生率(0%)、导管相关性血栓发生率(0%)及感染发生率(3.33%)均低于对照组(20.00%、13.33%、16.67%、23.33%)(P<0.05)。结论 隧道式PICC置管穿刺成功率高,能明显减轻穿刺点渗血程度,降低机械性静脉炎、导管相关性血栓及感染发生情况。 Objective To investigate the clinical effect of tunnel PICC implantation.Methods 60 cancer patients(requiring parenteral nutrition support or long-term intravenous chemotherapy)in Pingxiang Second People's Hospital from January 2020 to May 2022,were randomly divided into the study group(n=30)and the control group(n=30).The patients in the control group were treated with traditional PICC catheter,and the patients in the study group were treated with tunnel PICC implantation.Follow up the success of puncture and complications,comprehensively evaluate the clinical efficacy,and compare the differences between two different catheter placement methods.Results The success rate of one puncture in the study group(96.67%)was higher than that in the control group(80.00%)(P<0.05).The incidence of 48 hours'bleeding(3.33%),mechanical phlebitis(0%),catheter related thrombosis(0%)and infection(3.33%)in the study group were lower than those in the control group(20.00%,13.33%,16.67%,23.33%)(P<0.05).Conclusion The success rate of tunnel PICC catheterization is high,which can significantly reduce the degree of bleeding at the puncture point,and reduce the occurrence of mechanical phlebitis,catheter related thrombosis and infection.
