强心饮合补阳还五汤加减结合西医常规疗法治疗慢性心力衰竭的临床疗效及对左心室射血分数、脑钠肽水平、生活质量的影响分析

The clinical efficacy of modified Qiangxin decoction and Buyang Huanwu decoction combined with conventional Western medicine therapy in the treatment of chronic heart failure and its effects on LVEF,BNP level and quality of life

目的:探析强心饮合补阳还五汤加减结合常规西医治疗慢性心力衰竭患者的临床效果及对其左心室射血分数(Left Ventricular Ejection Fraction,LVEF)、脑钠肽(Brain Natriuretic Peptide,BNP)水平、生活质量的影响。方法:选取2019年12月-2021年6月于临沂市中心医院中医科接受治疗的慢性心力衰竭患者80例作为研究对象,按计算机分组法将所有患者随机分为研究组和对照组,每组40例,对照组接受常规西医治疗,研究组在此基础上采用强心饮合补阳还五汤加减治疗,观察两组患者的治疗有效率,对比其治疗前后LVEF、BNP水平变化情况,并采用36项简明健康生活状态量表(The Mos 36-item Short From Health Survey,SF-36)评估其生活质量。结果:研究组治疗后总有效率95.00%(38/40)高于对照组的80.00%(32/40),研究组治疗后LVEF和SF-36评分高于对照组而BNP水平低于对照组,差异有统计学意义(P<0.05)。结论:强心饮合补阳还五汤加减结合西医治疗慢性心力衰竭患者的疗效显著,可在改善患者临床症状及体征的同时改善其LVEF、BNP等心功能指标,且经中医配合治疗后患者生活质量可显著提高,此疗法具有较高临床应用价值。

Objective:To explore the clinical effect of modified Qiangxin decoction(强心饮)and Buyang Huanwu decoction(补阳还五汤)combined with conventional Western medicine therapy in the treatment of chronic heart failure and its effects on left ventricular ejection fraction(LVEF),brain natriuretic peptide(BNP)level and quality of life.Methods:A total of 80 patients with chronic heart failure who received treatment in the Department of Traditional Chinese Medicine of Linyi Central Hospital from December 2019 to June 2021 were selected as the study subjects.All the patients were randomly divided into the study group and the control group according to the computer grouping method,with 40 cases in each group.The control group received conventional Western medicine treatment,and the study group was treated with modified Qiangxin decoction and Buyang Huanwu decoction on the basis of the treatment in the control gruoup.The effective rate of treatment in the two groups was observed,the changes of LVEF and BNP levels before and after treatment were compared,and the quality of life in the two groups was assessed by SF-36.Results:The total effective rate in the study group after treatment was 95.00%(38/40),which was higher than that in the control group of 80.00%(32/40),the LVEF and the score of SF-36 in the study group were higher than those in the control group,and the level of BNP in the study group was lower than that in the control group,the difference was of statistical significance(P<0.05).Conclusion:modified Qiangxin decoction and Buyang Huanwu decoction combined with conventional Western medicine therapy has significant curative effect in the treatment of chronic heart failure,which can relieve the clinical symptoms and signs of patients and improve their LVEF,BNP and other cardiac function indicators,and the quality of life of patients can be significantly improved after the treatment of traditional Chinese medicine.This therapy has high clinical application value.
