针灸治疗偏头痛经验举隅

Experience for treating migraine by acupuncture

偏头痛是临床常见病证,其临床常表现为一侧头部疼痛反复发作。此病常伴有恶心、呕吐、对光及声音过度敏感等症状。中医学认为偏头痛属“头风”“脑风”等范畴,纵观历代医家,偏头痛的病因外感多责之于风、寒、湿、热,内伤多关乎气、血、痰、瘀、虚。随着生活节奏的加快,偏头痛的发病率也随之升高。目前防治中存在诸多问题,寻求有效治则治法是患者和社会的需要。安徽中医药大学第一附属医院潘宾副主任医师从事临床近二十年,总结出偏头痛病机不外乎“不通则痛”和“不荣则痛”两大类。潘副主任主张“首辨主病兼病,再辨证型经络,后给予施针中药”。临证中,他灵活运用“开四关”“热敏灸找敏感点”“先阴后阳”等特色针灸方法治疗,且用药时重视用川芎及虫类药,取得了良好疗效,形成治疗偏头痛的独特临证特色。文章通过案例,介绍潘宾副主任医师针灸治疗偏头痛经验,以期为临床参考。

Migraine is a common clinical disease,and its clinical manifestations are repeated attacks of pain in one side of the head.This disease is often accompanied by nausea,vomiting,hypersensitivity to light and sound,etc..TCM believes that migraine belongs to head wind,brain wind and other categories.Throughout the history of physicians,the causes of migraine are mainly due to wind,cold,damp and heat,while internal injuries are mostly related to Qi(气),blood,phlegm,blood stasis and deficiency.With the acceleration of the life pace,the incidence of migraine is also increased.PAN Bin,deputy director of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine,has been engaged in clinical work for nearly 20 years,and has summarized that the pathogenesis of migraine is nothing more than two reasons pain due to be obstructed and pain due to lack of nutrition.Deputy director PAN advocated that the main disease and concurrent disease should be identified first,and then the meridians and syndrome type should be identified,and then the acupuncture and TCM medicine should be given.In clinical practice,the characteristic acupuncture and moxibustion methods such as opening the four passes,heat sensitive moxibustion to find sensitive points,Yin(阴)before Yang(阳)were flexibly applied,and Chuanxiong(Rhizoma Chuanxiong),insect medicine were paid attention to when using the medicine,which achieved good effects.This article introduces deputy director PAN’s experience in treatment of migraine by acupuncture,in order to be a clinical reference.
