摘要
目的探讨电子支气管镜肺泡灌洗术治疗高原地区儿童重症细菌性肺炎的效果。方法回顾性分析2020年6月至2022年6月我院收治的163例儿童重症细菌性肺炎患儿的临床资料,根据治疗方法不同将其分为研究组(57例,电子支气管镜肺泡灌洗术+常规治疗)和对照组(106例,常规治疗)。比较两组的治疗效果。结果研究组的发热消失时间、咳嗽咳痰消失时间、电子计算机断层扫描(CT)结果明显好转时间、住院时间短于对照组,住院费用低于对照组,差异具有统计学意义(P<0.05)。入院第1天,两组的白细胞计数(WBC)、降钙素原(PCT)、C反应蛋白(CRP)水平比较,差异无统计学意义(P>0.05);入院第5、10天,研究组的WBC、PCT、CRP水平低于对照组,差异具有统计学意义(P<0.05)。研究组的临床症状反复、住院时间>14 d占比均低于对照组,差异具有统计学意义(P<0.05)。两组的不良事件发生情况比较,差异无统计学意义(P>0.05)。结论电子支气管镜肺泡灌洗术治疗高原地区儿童重症细菌性肺炎的效果确切,其可改善患儿症状,加快康复进程,减轻炎症反应,安全性高。
Objective To investigate the effect of electronic bronchoscopy alveolar lavage in the treatment of severe bacterial pneumonia in children in plateau area.Methods The clinical data of 163 children with severe bacterial pneumonia admitted in our hospital from June 2020 to June 2022 were retrospectively analyzed.According to different treatment methods,the children were divided into study group(57 cases,electronic bronchoscopy alveolar lavage+conventional treatment)and control group(106 cases,conventional treatment).The therapeutic effects of the two groups were compared.Results The fever disappearance time,cough and sputum disappearance time,time for significant improvement in computer tomography(CT)result and hospitalization time of the study group were shorter than those of the control group,the hospitalization cost was lower than that of the control group,and the differences were statistically significant(P<0.05).On the first day of admission,there were no significant differences in the white blood cell count(WBC),procalcitonin(PCT)and C-reactive protein(CRP)levels between the two groups(P>0.05);on the 5 th and 10 th day of admission,the WBC,PCT and CRP levels in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The proportions of recurrence of clinical symptoms,hospitalization time>14 d in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion Electronic bronchoscopy alveolar lavage is effective in the treatment of severe bacterial pneumonia in children in plateau area.It can improve the symptoms of children,speed up the rehabilitation process,and reduce the inflammatory response,with high safety.
