低温等离子手术治疗扁桃体腺样体肥大的有效性及安全性

Efficacy and safety of low-temperature plasma surgery in the treatment of tonsil adenoid hypertrophy

目的分析低温等离子手术治疗扁桃体腺样体肥大的有效性及安全性。方法选取2019年1月至2020年12月我院收治的84例扁桃体腺样体肥大患者为研究对象,随机将其分为常规组(42例,常规手术治疗)和观察组(42例,低温等离子手术治疗)。比较两组的治疗效果。结果观察组的治疗总有效率显著高于常规组(P<0.05)。观察组的术中出血量少于常规组,手术时长、伪膜脱落时间、住院时长短于常规组,术后6 h疼痛评分低于常规组(P<0.05)。术后3 d,观察组的超敏C-反应蛋白(hs-CRP)、降钙素原(PCT)、白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)水平低于常规组(P<0.05)。术后3 d,观察组的白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))高于常规组,白细胞分化抗原8阳性(CD8^(+))低于常规组(P<0.05)。术后6、12个月,观察组的呼吸暂停指数、呼吸暂停低通气指数、低通气指数、与呼吸事件相关微觉醒指数低于常规组,最低血氧饱和度高于常规组(P<0.05)。结论低温等离子手术治疗扁桃体腺样体肥大患者的效果理想。

Objective To analyze the efficacy and safety of low-temperature plasma surgery in the treatment of tonsil adenoid hypertrophy.Methods A total of 84 patients with tonsil adenoid hypertrophy admitted in our hospital from January 2019 to December 2020 were selected as the research objects and randomly divided into conventional group(42 cases,routine surgical treatment)and observation group(42 cases,low-temperature plasma surgery treatment).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the conventional group(P<0.05).The intraoperative blood loss in the observation group was less than that in the conventional group,the operation time,pseudomembrane shedding time and hospitalization time were shorter than those in the conventional group,and the pain score at 6 h after operation was lower that in the conventional group(P<0.05).At 3 d after operation,the levels of high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),interleukin-1β(IL-1β)and interleukin-8(IL-8)in the observation group were lower than those in the conventional group(P<0.05).At 3 d after operation,the cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the observation group were higher than those in the conventional group,while the cluster of differentiation 8 positive(CD8^(+))was lower than that in the conventional group(P<0.05).At 6,12 months after operation,the apnea index,apnea hypopnea index,hypopnea index and microarousal index related to respiratory events in the observation group were lower than those in the conventional group,while the lowest oxygen saturation was higher than that in the conventional group(P<0.05).Conclusion The low-temperature plasma surgery in the treatment of patients with tonsil adenoid hypertrophy has a ideal effect.
