摘要
目的探讨亚低温联合系统化急救护理对窒息新生儿动脉血乳酸水平及乳酸清除率的影响。方法选取2020年1月至2023年1月我院收治的50例窒息新生儿作为研究对象,按照随机数字表法将其分为对照组(20例,常规护理)及研究组(30例,亚低温联合系统化急救护理)。比较两组的护理效果。结果研究组的抢救时间、住院时间短于对照组,抢救后5 min Apgar评分高于对照组(P<0.05)。研究组的90 s内复苏率高于对照组(P<0.05)。干预5 min后,研究组的心率、呼吸频率高于对照组(P<0.05)。干预10 d后,研究组的新生儿神经行为测定量表(NBNA)评分高于对照组(P<0.05)。干预6 h后,研究组的动脉血乳酸水平低于对照组,乳酸清除率高于对照组(P<0.05)。结论亚低温联合系统化急救护理在新生儿窒息中的应用效果显著,可缩短抢救时间及住院时间,提高抢救后5 min Apgar评分及90 s内复苏率,改善患儿的心率、呼吸频率及神经功能,降低动脉血乳酸水平。
Objective To explore the effects of mild hypothermia combined with systematic emergency care on arterial blood lactate level and lactate clearance rate in asphyxiated newborns.Methods From January 2020 to January 2023,50 newborns with asphyxia admitted in our hospital were selected as the research objects.According to the random number table method,the newborns were divided into control group(20 cases,routine nursing)and study group(30 cases,mild hypothermia combined with systematic emergency care).The nursing effects of the two groups were compared.Results The rescue time and hospitalization time of the study group were shorter than those of the control group,and the Apgar score at 5 min after rescue was higher than that of the control group(P<0.05).The resuscitation rate within 90 s in the study group was higher than that in the control group(P<0.05).After 5 min of intervention,the heart rate and respiratory rate of the study group were higher than those of the control group(P<0.05).After 10 d of intervention,the Neonatal Behavioral Neurological Assessment(NBNA)score of the study group was higher than that of the control group(P<0.05).After 6 h of intervention,the arterial blood lactate level in the study group was lower than that in the control group,and the lactate clearance rate was higher than that in the control group(P<0.05).Conclusion The mild hypothermia combined with systematic emergency care in neonatal asphyxia has significant application effect,it can shorten the rescue time and hospitalization time,improve the Apgar score at 5 min after rescue and recovery rate within 90 s,improve the heart rate,respiratory rate and neurological function of children,and reduce the arterial blood lactate level.
