亚临床甲减与2型糖尿病患者TyG相关性研究

Relationship between Subclinical Hypothyroidism and TyG in Patients with Type 2 Diabetes

目的:探讨亚临床甲减对2型糖尿病患者甘油三酯—葡萄糖指数(TyG)的影响。方法:选择2019年1月—2020年12月在我院住院的2型糖尿病患者222例,按照甲状腺功能分为甲状腺功能正常组(138例)和亚临床甲减组(84例)。收集两组患者一般资料及空腹血糖、血生化、血脂、甲状腺功能等相关指标,计算TyG,使用线性回归分析甲状腺功能对TyG的影响。结果:(1)两组人群组间男女比例、体重、BMI、糖化血红蛋白、尿酸、甘油三酯、HDL-L、胱抑素C、FPG、TyG比较差异有统计学意义(P<0.05);(2)线性回归分析提示LnFT 4、LnT 4、LnTSH与TyG独立相关(P均<0.05),多因素分析提示LnTSH与TyG独立相关(P<0.05);(3)按甲状腺功能分类,线性回归分析提示亚临床甲减是TyG升高的独立危险因素(P<0.05),多因素分析提示在总体人群和男性中亚临床甲减是TyG升高独立危险因素(P<0.05)。结论:亚临床甲减与2型糖尿病患者TyG密切相关,其可能加重糖尿病患者的胰岛素抵抗。

Objective:To investigate the impact of subclinical hypothyroidism on TyG in patients with type 2 diabetes.Methods:222 patients with type 2 diabetes hospitalized in our hospital from January 2019 to December 2020 were selected,and were divided into normal thyroid function group(138 cases)and subclinical hypothyroidism group(84 cases)according to thyroid function.The general information and test indexes such as fasting blood glucose,blood biochemistry,blood lipid and thyroid function were collected,TyG was calculated,and the relationship between thyroid function and TyG was analyzed by linear regression analysis.Results:(1)There were statistically significant differences in the ratio of male to female,body weight,BMI,glycated hemoglobin,uric acid,triglyceride,HDL-C,cystatin C,FPG,and TyG between the two groups.(2)Simple linear regression analysis showed that LnFT 4,LnT 4 and LnTSH were independently correlated with TyG(P<0.05),while multivariate analysis showed that LnTSH was independently correlated with TyG in diabetic patients(P<0.05).(3)Linear regression analysis showed that subclinical hypothyroidism was the independent risk factor for TyG(P<0.05).Conclusion:Subclinical hypothyroidism closely relate to TyG in type 2 diabetes mellitus,which may aggravate insulin resistance.
