摘要
目的分析基于量化评估下的早期四级康复训练在重症加强护理病房(ICU)机械通气患者中的应用价值。方法120例ICU机械通气患者,采取随机数字表法分为对照组与观察组,各60例。对照组给予常规早期康复训练,观察组给予基于量化评估下的早期四级康复训练。比较两组患者康复训练前后运动功能评分、认知功能评分、肺功能指标水平以及康复训练期间并发症(呼吸机相关性肺炎、ICU获得性肌无力、压疮、下肢静脉血栓)发生情况。结果康复训练后,观察组患者运动功能评分(70.29±5.24)分、认知功能评分(27.81±2.09)分均高于对照组的(52.45±4.31)、(22.75±2.12)分,差异具有统计学意义(P<0.05)。康复训练后,观察组患者用力肺活量(FVC)(2.76±0.32)L、第1秒用力呼气容积与用力肺活量比值(FEV1/FVC)(76.45±8.33)%均高于对照组的(2.32±0.46)L、(67.15±9.02)%,差异具有统计学意义(P<0.05)。观察组患者康复训练期间并发症发生率为3.33%,与对照组的6.67%比较,差异无统计学意义(P>0.05)。结论基于量化评估下的早期四级康复训练应用于ICU机械通气患者中,可有效提高患者的运动及认知功能,改善其肺功能,且在康复训练期间并发症发生率低,安全性较高。
Objective To analyze the application value of early level 4 rehabilitation training based on quantitative evaluation in patients with mechanical ventilation in intensive care unit(ICU).Methods A total of 120 patients with mechanical ventilation in ICU were divided into control group and observation group according to the random numerical table,with 60 cases in each group.The control group was given routine early rehabilitation training,and the observation group was given early level 4 rehabilitation training based on quantitative evaluation.Both groups were compared in terms of motor function score,cognitive function score and pulmonary function indexes before and after rehabilitation training,as well as the occurrence of complications(ventilator-associated pneumonia,ICU acquired weakness,pressure sore,lower limb venous thrombosis)during rehabilitation training.Results After rehabilitation training,the motor function score of(70.29±5.24)points and cognitive function score of(27.81±2.09)points in the observation group were higher than those of(52.45±4.31)and control group(22.75±2.12)points in the control group,and the differences were statistically significant(P<0.05).After rehabilitation training,the forced vital capacity(FVC)of(2.76±0.32)L and ratio of forced expiratory volume in one second to FVC(FEV1/FVC)of(76.45±8.33)%in the observation group were higher than those of(2.32±0.46)L and(67.15±9.02)%in the control group,and the differences were statistically significant(P<0.05).The incidence of complications during rehabilitation in the observation group was 3.33%,and the difference was not statistically significant when compared with that of 6.67%in the control group(P>0.05).Conclusion Early level 4 rehabilitation training based on quantitative evaluation can effectively improve the motor function and cognitive function and pulmonary function of patients with mechanical ventilation in ICU,and the incidence of complications during rehabilitation training is low.So it has high safety.
