摘要
目的探究血清miR-382-5p、降钙素原(PCT)、淀粉样蛋白A(SAA)和C反应蛋白(CRP)与早产儿肺部疾病相关性研究。方法选取2016年1月至2021年1月入住保定市第一中心医院的157例早产儿作为研究对象,其中无肺部疾病的新生儿38例(对照组),有肺部疾病的新生儿119例,并将患有肺部疾病的新生儿分为新生儿呼吸窘迫综合征(NRDS)组(39例)、支气管肺发育不良(BPD)组(40例)、肺动脉高压(PPHN)组(40例),Pearson法分析miR-382-5p与PCT、SAA和CRP水平的相关性,Logistic回归分析早产儿患肺部疾病的影响因素,受试者工作特征(ROC)曲线分析miR-382-5p的诊断价值。结果与对照组比较,NRDS、BPD、PPHN组患儿血清miR-382-5p表达水平均显著降低,而PCT、SAA和CRP水平均显著上升,差异具有统计学意义(F值分别为216.047、83.917、183.164、124.093,P<0.05);Pearson相关分析显示miR-382-5p表达水平与PCT、SAA和CRP水平均呈负相关(r值介于-0.583~-0.450之间,P<0.05);Logistic回归分析显示低表达miR-382-5p,PCT、SAA、CRP高水平及低出生体重均是早产儿患肺部疾病的独立危险因素(OR值分别为4.301、1.243、7.247、3.421、3.469,P<0.05);ROC分析显示血清miR-382-5p的曲线下面积为0.858,敏感度和特异度分别为90.42%和85.63%。结论miR-382-5p在患肺部疾病的早产儿血清中低表达,PCT、SAA、CRP高表达,均与早产儿肺部疾病相关,可作为辅助早产儿肺部疾病诊断的标志物。
Objective To explore the correlation between serum miR-382-5p,procalcitonin(PCT),serum amyloid A(SAA),C-reactive protein(CRP)and pulmonary diseases in preterm infants.Methods A total of 157 premature infants admitted to Baoding First Hospital from January 2016 to January 2021 were selected as the research objects,including 38 neonates without lung diseases(control group)and ll9 neonates with lung diseases.The neonates with lung diseases were divided into neonatal respiratory distress syndrome(NRDS)group(39 cases),bronchopulmonary dysplasia(BPD)group(40 cases),and persistent pulmonary hypertension of the newborn(PPHN)group(40 cases).Pearson method was used to analyze the correlation between miR-382-5p and PCT,SAA,and CRP levels.Logistic regression was used to analyze the influencing factors of lung diseases in preterm infants,and the receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of miR-382-5p.Results Compared with the control group,the expression levels of miR-382-5p in serum of NRDS,BPD and PPHN groups were significantly decreased,while the levels of PCT,SAA and CRP were significantly increased,with statistical significance(F=216.047,83.917,183.164 and 124.093,respectively,P<0.05).Pearson correlation analysis showed that the expression level of miR-382-5p was negatively correlated with the levels of PCT,SAA and CRP(r value ranged from-0.583 to-0.450,P<0.05).Logistic regression analysis showed that low expression of miR382-5p,PCT,SAA,CRP and low birth weight were independent risk factors for lung disease in preterm infants(OR=4.301,1.243,7.247,3.421 and 3.469,respectively,P<0.05).ROC analysis showed that the area under the curve of serum miR-382-5p was 0.858,and the sensitivity and specificity were 90.42%and 85.63%,respectively.Conclusion The miR-382-5p has low expression and high expression of PCT,SAA and CRP in the serum of preterm infants with lung diseases,which are related to lung diseases in preterm infants and can be used as markers to assist the diagnosis of lungdiseases inpreterm infants.
