扁桃体等离子切除术后出血的影响因素分析

Analysis of Influencing Factors of Postoperative Bleeding after Tonsil Plasma Resection

目的:探讨扁桃体等离子切除术后出血的影响因素。方法:选取2022年7月-2023年3月惠州市中心人民医院收治的扁桃体切除术患者745例作为研究对象。根据患者术后是否发生出血分为出血组与非出血组。回顾性分析两组患者性别、年龄、体重指数(BMI)、术前诊断、扁桃体嵌入程度、术前中性粒细胞与淋巴细胞比值、主刀医师、术中出血止血方法、创面白膜形成情况、术后疼痛等临床指标,并对其进行比较分析。结果:出血组与非出血组年龄、BMI、术前诊断、扁桃体嵌入程度、术中出血止血方法、创面白膜形成情况、术后疼痛比较,差异有统计学意义(P<0.05);年龄15~60岁、术中出血采用电凝止血、创面白膜形成情况差、术后重度疼痛是扁桃体术后出血的独立危险因素。结论:年龄、术中出血止血方法、创面白膜形成情况及术后疼痛为扁桃体术后出血的主要影响因素。

Objective:To investigate the influencing factors of postoperative bleeding after tonsil plasma resection.Methods:Seven hundred and forty-five tonsillectomy patients admitted to Huizhou Central People's Hospital from July 2022 to March 2023 were selected as study subjects.The patients were divided into bleeding group and non-bleeding group according to whether they had postoperative bleeding or not.Clinical indexes such as gender,age,body mass index(BMI),preoperative diagnosis,degree of tonsil embedding,preoperative neutrophil-to-lymphocyte ratio,attending surgeon,intraoperative hemorrhagic hemostasis method,traumatic white film formation,postoperative pain and other clinical indexes of patients in the two groups were retrospectively analyzed.Results:Age,BMI,preoperative diagnosis,degree of tonsil embeddedness,intraoperative bleeding hemostasis method,traumatic white film formation,and postoperative pain were compared between the bleeding group and the non-bleeding group,the difference was statistically significant(P<0.05);Age 15-60 years,intraoperative bleeding by electrocoagulation hemostasis,poor traumatic white film formation,and severe postoperative pain were independent risk factors for postoperative tonsillar hemorrhage.Conclusion:Age,intraoperative bleeding hemostasis method,traumatic white film formation and postoperative pain are the main influencing factors for postoperative tonsil bleeding.
