集成磁共振成像定量参数鉴别乳腺良恶性病变的价值

摘要 目的:探讨集成磁共振成像定量参数(syMRI)鉴别乳腺良恶性病变的价值。方法:选取2019年1月—2021年6月龙口市人民医院收治的乳腺肿块患者43例为观察对象,对患者进行常规MRI扫描,在增强前后均配合集成磁共振成像(syMRI)序列完成扫描,在后期处理工作站生成影像并完成感兴趣体积勾画,得到各项参数,计算得到病灶的表观弥散系数、时间信号强度曲线。对乳腺良恶性病变各项参数进行比较,评价syMRI的临床价值。结果:经手术病理检验后确认纳入患者有13例为良性病变,30例为恶性病变。良性病变患者syMRI扫描参数T1pre、T2pre、T1^(-1)、T2^(-1)数值高于恶性病变患者,差异有统计学意义(P<0.05)。syMRI增强前参数T1pre诊断效能最高,曲线下面积为0.869,最佳界值取为1282.94 ms时,T1pre对乳腺恶性病变诊断的灵敏度与特异度分别为76.9%、93.3%。结论:syMRI定量参数中的数据可作为鉴别乳腺良恶性病变的参考依据。 Objective:To explore the value of integrated magnetic resonance imaging quantitative parameters(syMRI)to identify benign and malignant breast lesions.Methods:Forty-three patients with breast masses admitted to Longkou People's Hospital from January 2019 to June 2021 were selected for observation.Conventional MRI scanning was performed on the patients,and the scanning was completed with integrated magnetic resonance imaging(syMRI)sequences both before and after enhancement,and the image was generated and the volume of interest outlining was completed at the post-processing workstation to obtain the parameters,and the apparent diffusion coefficient and the time-signal intensity curves of the lesions were calculated.The parameters of benign and malignant breast lesions were compared to evaluate the clinical value of syMRI.Results:Thirteen cases of benign lesions and thirty cases of malignant lesions were confirmed to be benign in the included patients after surgical pathologic examination;The values of syMRI scanning parameters T1pre,T2pre,T1^(-1),T2^(-1)were higher in patients with benign lesions than in patients with malignant lesions,and the difference was statistically significant(P<0.05);The syMRI pre-enhancement parameter T1pre had the highest diagnostic efficacy,with an area under the curve of 0.869,and when the optimal cut-off value was taken as 1282.94 ms,the sensitivity and specificity of T1pre for the diagnosis of malignant lesions in the breast were 76.9%and 93.3%,respectively.Conclusion:The data in the quantitative parameters of syMRI can be used as a reference basis for identifying benign and malignant breast lesions.
