超声与X线引导下脉冲射频治疗50例神经根型颈椎病的疗效比较

Comparision of efficacy of ultrasound and X-ray guided pulsed radiofrequency in the treatment of 50 cases of cervical spondylotic radiculopathy

[背景]比较超声(US)与X线引导下选择性神经根脉冲射频治疗神经根型颈椎病(CSR)的疗效.[病例报告]选择2019年3月—2021年3月间就诊于延边大学附属医院疼痛科的CSR患者50例,采用随机数字表法分为X线引导组和US引导组,每组各为25例,记录并比较术前和术后1 d、4、12、24、48周时的NRS评分、AIS评分、有效率及不良反应发生率.两组患者术后1 d、4、12、24、48周时的NRS和AIS评分均较术前显著降低(P<0.05,P<0.01);与X线引导组比较,US引导组NRS和AIS评分明显降低(P<0.05),并发症发生率亦显著降低(P<0.01).[讨论] US引导下选择性神经根脉冲射频治疗CSR较X线引导技术准确、安全、有效.

BACKGROUND To compare the clinical efficacy of selective nerve root pulsed radiofrequency under the guidance of ultrasound(US) or X-ray for cervical spondylotic radiculopathy(CSR).CASE REPORTS Fifty CSR patients admitted to the pain department of the affiliated hospital of Yanbian university from March 2019 to March 2021 were selected and divided into X-ray guided group and US-guided group by random number table method,with 25 cases in each group.NRS score,AIS score,effective rate and adverse reactions were recorded and compared before operation and at 1 day,4,12,24,48 weeks after operation.The NRS and AIS scores of the two groups at 1 day,4,12,24 and 48 weeks after operation were significantly lower than those before operation(P<0.05,P<0.01).Compared with the X-ray guided group,the NRS and AIS scores were significantly lower in the USguided group(P<0.05),and the incidence of complications was also significantly lower(P<0.01).DISCUSSION Selective nerve root pulsed radiofrequency under the guidance of US for the treatment of CSR is more precise,safe and effective than X-ray guided technology.
