不同压迫方式对腹腔镜腹股沟疝术后并发症的影响:基于倾向性评分匹配法

The effect of different compression methods on postoperative complications of laparoscopic inguinal hernia repair based on propensity score matching

目的:探讨盐袋、冰敷、疝气带压迫方式对腹腔镜腹股沟疝修补术后并发症的影响。方法:将2020年9月1日至2022年6月1日行腹腔镜腹股沟疝修补术的患者分为盐袋组、冰敷组、疝气带组,两两分组,对比各组患者的基线信息,再进行倾向性得分匹配,平衡其他混杂因素对术后并发症的影响。结果:倾向性评分匹配后,各组基线信息差异无统计学意义。盐袋组与冰敷组血清肿、复发发生率差异无统计学意义,但冰敷组术后疼痛发生率低、疼痛评分低、疼痛时间短。盐袋组与疝气带组均未发生复发,血清肿发生率差异无统计学意义,疝气带组疼痛发生率低、疼痛评分低、疼痛时间更短。冰敷组与疝气带组复发、血清肿发生率差异无统计学意义,疝气带组疼痛发生率低、疼痛评分低、疼痛时间更短。结论:腹腔镜腹股沟疝手术后采用疝气带压迫在减轻术后疼痛方面优于盐袋、冰敷压迫。

Objective:To investigate the influence of salt bag,ice compress and hernia band compression on postoperative c omplications of laparoscopic inguinal hernia repair.Methods:The patients who underwent laparoscopic inguinal hernia repair from Sep.1,2020 to Jun.1,2022 were divided into three groups:salt bag group,ice compress group and hernia band group.The baseline data of each group were compared,and the propensity score matching was performed to balance the influence of other confounding factors on postoperative complications.Results:After propensity score matching,there was no statistically significant difference in baseline data a mong the 3 groups.No postoperative infection was found in each group,so it was not included in the statistical analysis.There was no significant difference in seroma and recurrence between the salt bag group and the ice compress group,but the ice compress group had less postoperative pain incidence,lower pain score and less pain time than the salt bag group.There was no recurrence in the salt bag group and the hernia band group,and there was no significant difference in the incidence of seroma.However,the hernia band group had less pain incidence,lower pain score and shorter pain time.There was no significant difference in recurrence and seroma between the ice compress group and the hernia band group.The hernia band group had less postoperative pain incidence,lower pain score and shorter pain time than the ice compress group.Conclusions:The effect of hernia band compression on reducing the postoperative pain is better than that of salt bag and ice compress compression.
