针灸联合丁苯酞治疗老年脑卒中的疗效及对D-二聚体和血液流变学的影响

Effect of acupuncture and moxibustion combined with butylphthalide on senile stroke and its influence on D-dimer and hemorheology

目的探讨针灸联合丁苯酞治疗老年脑卒中的疗效及对D-二聚体和血液流变学的影响。方法选取2018年3月至2019年12月本院收治的120例老年脑卒中患者作为研究对象,采用随机数字表法分为对照组与观察组,各60例。对照组采用丁苯酞治疗,观察组在对照组基础上加用针灸治疗,比较两组临床疗效、D-二聚体及血液流变学指标。结果观察组治疗总有效率为93.33%,明显高于对照组的81.67%,差异有统计学意义(P<0.05)。治疗前,两组D-二聚体水平比较差异无统计学意义;治疗后,两组D-二聚体水平明显低于治疗前,且观察组明显低于对照组,差异有统计学意义(P<0.05)。治疗前,两组全血黏度、纤维蛋白原水平及血细胞比容比较差异无统计学意义;治疗后,两组全血黏度、纤维蛋白原水平及血细胞比容均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论针灸联合丁苯酞治疗老年脑卒中的疗效显著,可改善患者D-二聚体及血液流变学指标,值得临床推广应用。

Objective To study the efficacy of acupuncture and moxibustion combined with butylphthalide in the treatment of senile cerebral apoplexy and its effect on D-dimer and hemorheology.Methods A total of 120 elderly stroke patients admitted to our hospital from March 2018 to December 2019 were selected as the research subjects,and they were divided into the control group and the observation group according to random number table methos,with 60 cases in each group.The control group was treated with butylphthalide,and the observation group was treated with acupuncture on the basic of control group,the clinical efficacy,D-dimer and hemorheology indexs were compared between the two groups.Results The total effective rate in the observation group was 93.33%,which was significantly higher than 81.67%in the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in D-dimer level between two groups;after treatment,the D-dimer level of the two groups were significantly lower than that before treatment,and the the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in whole blood viscosity,fibrinogen levels and hematocyte specific volume between two groups;after treatment,the levels of whole blood viscosity,fibrinogen levels and hematocyte specific volume of the two groups were lower than before treatment,and the the observation group was significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion Acupuncture and moxibustion combined with butylphthalide in the treatment of elderly stroke is effective,can improve the D-dimer and hemorheological indexes,which is worthy of clinical promotion and application.
