摘要
目的探讨血清肿瘤坏死因子-α(TNF-α)、白细胞介素-18(IL-18)、转化生长因子-β(TGF-β)在老年高血压脑小血管病(CSVD)患者中的水平变化及临床意义。方法选取2018年8月至2020年12月本院收治的120例高血压CSVD患者作为研究对象,按照患者病情程度分为对照组(轻度高血压CSVD)与观察组(中度高血压CSVD),各60例。比较两组患者蒙特利尔认知评估量表(MoCA)评分,血清TNF-α、IL-18、TGF-β、同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)及总胆固醇(TC)水平。结果观察组执行能力、记忆力、语言、定向等MoCA评分均低于对照组,差异有统计学意义(P<0.05);观察组血清TNF-α、IL-18、Hcy、hs-CRP水平均高于对照组,血清TGF-β水平低于对照组,差异有统计学意义(P<0.05);两组血清TC水平比较差异无统计学意义。结论血清TNF-α、IL-18、TGF-β水平的变化对于诊断老年高血压CSVD患者具有重要临床意义,其水平的高低可反映患者病情严重程度,值得临床借鉴。
Objective To analysethe Changes of serum tumor necrosis factor-α(TNF-α),interleukin-18(IL-18)and transforming growth factor-β(TGF-β)levels and their clinical significance in elderly patients with hypertensive cerebrovascular disease(CSVD).Methods 120 patients with CSVD admitted in our hospital from August 2018 to December 2020 were selected as the research subjects,and they were divided into the control group(mild hypertension CSVD)and the observation group(moderate hypertension CSVD)according to their severity,with 60 cases in each group.Montreal cognitive assessment(MoCA)scores,serum TNF-α,IL-18,TGF-β,Hcy,hs-CRP and TC levels were compared between the two groups.Results The MoCA scores of execution ability,memory,language,orientationin the observation group were lower than those in the control group,the differences were statistically significant(P<0.05);serum TNF-α,IL-18,Hcy and hs-CRP levels in the observation group were higher than those in the control group,serum TGF-βlevel was lower than that in the control group,the differences were statistically significant(P<0.05);there was no significant difference in serum TC levels between the two groups.Conclusion The level of serum TNF-α,IL-18 and TGF-βcan reflect the severity of elderly hypertensive CSVD,which has important clinical significance for the diagnosis of disease and deserves clinical reference.
