经鼻高流量湿化治疗仪配合支气管镜肺泡灌洗在重症肺炎合并低氧血症患者中的应用效果分析

Analysis of the Application Effect of Nasal High Flow Humidification Therapy Instrument Combined with Bronchoscopy Alveolar Lavage in Patients with Severe Pneumonia Complicated with Hypoxemia

目的:探究经鼻高流量湿化治疗仪配合支气管镜肺泡灌洗在重症肺炎合并低氧血症患者中的应用效果。方法:选取2021年5月~2022年5月本院收治的98例重症肺炎合并低氧血症患者,采用简单随机化分组方式,将患者分为两组,分别为对照组(49例)以及观察组(49例)。对照组患者接受支气管镜肺泡灌洗治疗,观察组患者接受支气管镜肺泡灌洗联合经鼻高流量湿化治疗仪治疗,比较两组患者以下指标:肺功能指标、血气分析指标、急性生理学与慢性健康状况Ⅱ评分、重症监护室入住率与病死率、并发症发生率。结果:对比两组患者肺功能指标,观察组治疗后高于对照组(P<0.05)。对比两组患者血气分析指标,观察组氧分压、血氧饱和度(SaO2)水平高于对照组,二氧化碳分压低于对照组(P<0.05)。对比两组患者急性生理学与慢性健康状况Ⅱ评分,观察组治疗后低于对照组(P<0.05)。对比两组患者重症监护室入住率与病死率,观察组低于对照组(P<0.05)。对比两组患者并发症发生率,观察组低于对照组(P<0.05)。结论:对重症肺炎合并低氧血症患者实行支气管镜肺泡灌洗联合经鼻高流量湿化治疗仪治疗,能显著改善患者肺功能指标,改善患者血气分析指标,降低患者急性生理学与慢性健康状况Ⅱ评分,减少患者重症监护室入住率、病死率与并发症发生率。

Objective:To explore the application effect of nasal high flow humidification therapy instrument combined with bronchoscopy alveolar lavage in patients with severe pneumonia complicated with hypoxemia.Methods:98 patients with severe pneumonia combined with hypoxemia admitted to our hospital from May 2022 to May 2022 were randomly divided into two groups:the control group(49 cases)and the observation group(49 cases).The patients in the control group were treated with bronchoscopy alveolar lavage,and the patients in the observation group were treated with bronchoscopy alveolar lavage combined with nasal high flow humidification therapy apparatus.The following indicators of the two groups were compared:lung function indicators,blood gas analysis indicators,acute physiology and chronic health status II score,ICU occupancy rate and mortality,and complication rate.Results:Comparing the lung function indicators of the two groups of patients,the observation group was higher than the control group after treatment(P<0.05).By comparing the blood gas analysis indexes of the two groups,the levels of PaO2 and oxygen saturation in the observation group were higher than those in the control group,and the partial pressure of carbon dioxide(PaCO2)was lower than that in the control group(P<0.05).Comparing the acute physiology and chronic health status II scores of two groups of patients,the observation group showed lower scores after treatment compared to the control group(P<0.05).Comparing the ICU occupancy rate and mortality rate between the two groups of patients,the observation group was lower than the control group(P<0.05).Compared with the two groups of patients,the incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:The combination of bronchoscopy alveolar lavage and nasal high flow humidification therapy instrument can significantly improve the lung function indicators of patients with severe pneumonia complicated with hypoxemia,improve the blood gas analysis indicators of patients,reduce the acute physiology and chronic health status II score of patients,and reduce the occupancy rate,mortality rate and complication rate of patients in ICU.
