不同分级原发性高血压血压变异性与空腹甘油三酯血糖指数及内皮功能的相关性分析 被引量:2

摘要 目的探讨不同级别原发性高血压患者血压变异性与空腹甘油三酯血糖(TyG)指数及内皮功能[一氧化氮(NO)和内皮素-1(ET-1)]的相关性。方法选择2018年1月至2020年12月在金昌市第一人民医院就诊的原发性高血压患者229例,根据患者血压水平分为三组,高血压1级组48例,高血压2级组99例,高血压3级组82例,比较三组高血压患者血压变异性和TyG指数及内皮功能指标的关系,应用Pearson相关性分析血压变异性指标与TyG指数、NO和ET-1的相关性。结果不同高血压分级组TyG指数、NO和ET-1水平差异无统计学意义(P>0.05);不同高血压分级组24 h收缩压标准差(24 h SSD)、24 h舒张压标准差(24 h DSD)、白天收缩压标准差(dSSD)均差异具有统计学意义(F=5.014、3.154、3.341,P<0.05);白天舒张压标准差(dDSD)、夜间收缩压标准差(nSSD)、夜间舒张压标准差(nDSD)均差异无统计学意义(P>0.05);24 h SSD在高血压3级组与NO正相关(P<0.05),其它高血压组别与TyG指数、NO和ET-1均无相关性(P>0.05);24 h DSD、dSSD在不同高血压级别与TyG指数、NO和ET-1均无相关性(P>0.05);dDSD在高血压2级组与NO和ET-1显著正相关(P<0.05),高血压3级组与NO正相关(P<0.05);nSSD在高血压1级组与TyG指数正相关(P<0.05),在高血压2级组与NO和ET-1显著正相关(P<0.05);nDSD在高血压2级组与NO呈显著正相关(P<0.05),在其它高血压组别与TyG指数、NO和ET-1均无相关性(P>0.05)。结论不同高血压分级患者血压变异性差异显著,TyG指数、NO及ET-1可能是原发性高血压血压变异的危险因素。 Objective To investigate the correlation between blood pressure variability and triglyceride glucose(TyG)index as well as endothelial function[nitric oxide(NO)and endothelin-1(ET-1)]in patients with essential hypertension with different grades.Methods A total of 229 patients with essential hypertension in the First People’s Hospital of Jinchang from January 2018 to December 2020 were chosen.According to the blood pressure level,patients were divided into hypertension grade 1 group(48 cases),hypertension grade 2 group(99 cases),and hypertension grade 3 group(82 cases).The relation of blood pressure variability with TyG index and endothelial function including NO and ET-1 in three groups was compared.Pearson correlation analysis was used to analyze the correlation between blood pressure variability and TyG index,NO and ET-1.Results There was no significant difference in TyG index,NO and ET-1 among different hypertension grade groups(P>0.05).There were significant differences in 24 h systolic blood pressure standard deviation(24 h SSD),24 h diastolic blood pressure standard deviation(24 h DSD)and daytime systolic blood pressure standard deviation(dSSD)among different hypertension grade groups(F=5.014,3.154,3.341;P<0.05).There was no significant difference in daytime diastolic blood pressure standard deviation(dDSD),night systolic blood pressure standard deviation(nSSD)and night diastolic blood pressure standard deviation(nDSD)among them(P>0.05).Twenty-four h SSD in hypertension grade 3 group was positively correlated with NO(P<0.05).There was no correlation in TyG index,NO or ET-1 with other hypertension grade groups.Twenty-four h DSD and dSSD in different hypertension grade groups were not correlated with TyG index,NO and ET-1(P>0.05).dDSD was significantly positively correlated with NO and ET-1 in hypertension grade 2 group(P<0.05);and also positively correlated with NO in hypertension grade 3 group(P<0.05).NSSD was positively correlated with TyG index in hypertension grade 1 group(P<0.05),and significantly positively correlated with NO and ET-1 in hypertension grade 2 group(P<0.05).nDSD was significantly positively correlated with NO in hypertension grade 2 group(P<0.05),and had no correlation with TyG index,NO and ET-1 among other hypertension grade groups(P>0.05).Conclusion There are significant differences in blood pressure vTyG index,NO and ET-1levels are likely the risk factors for blood pressure variability in essential hypertension.
