长链非编码RNA-Safe水平与急性心肌梗死后左心室重构的相关性

摘要 目的探讨长链非编码RNA(lncRNA)-Safe与急性心肌梗死(AMI)患者左心室重构(LVR)的关系。方法选择2020年1月至2021年1月绍兴文理学院附属医院确诊的95例AMI患者作为AMI组,及同期一般资料相匹配的50例健康者作为对照组,收集研究对象一般资料,检测血清lncRNA-Safe表达水平;根据是否发生LVR将AMI患者分为非LVR组(53例)和LVR组(42例),分析非LVR组和LVR组患者临床资料及lncRNA-Safe表达水平差异,使用Pearson法分析lncRNA-Safe水平与LVR相关指标的相关性,使用受试者工作特征曲线(ROC)分析lncRNA-Safe对LVR的诊断价值。结果AMI组lncRNA-Safe水平显著高于对照组(t=15.736,P<0.01),LVR组lncRNA-Safe水平高于非LVR组(t=5.290,P<0.01)。LVR组患者左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)高于非LVR组,而左心室射血分数(LVEF)、左心室重构指数(LVRI)均低于非LVR组(t=6.363、3.152、5.759、2.187,P<0.05)。Pearson分析结果显示,lncRNA-Safe与LVESV、LVEDV呈正相关(r=0.716、0.701,P<0.01),而与LVEF、LVRI呈负相关(r=-0.552、-0.442,P<0.01)。lncRNA-Safe诊断AMI患者LVR发生的曲线下面积为0.765(95%CI=0.667~0.864),敏感度为64.30%,特异度为86.80%。结论lncRNA-Safe在AMI患者血清中高表达,其水平与LVR的发生存在相关性,可作为诊断AMI患者LVR发生的血清标志物。 Objective To explore the relationship between long non-coding RNA(lncRNA)-Safe and left ventricular remodeling(LVR)in patients with acute myocardial infarction(AMI).Methods Ninety-five AMI patients diagnosed in the Affiliated Hospital of Shaoxing University of Arts and Sciences from January 2020 to January 2021 were selected as AMI group,and 50 healthy people whose general data matched the patients in AMI group in the corresponding period were selected as the control group.The general information of the research subjects was collected,and the expression level of serum lncRNA-Safe was detected.According to whether LVR occurred,AMI patients were grouped into non-LVR group(53 cases)and LVR group(42 cases).The differences in clinical data and the expression levels of lncRNA-Safe were analyzed between the non-LVR group and the LVR group,the correlation between lncRNA-Safe and relevant indicators of LVR was analyzed by the Pearson method,and the diagnostic value of lncRNA-Safe for LVR was analyzed by receiver operating characteristic curve(ROC).Results The level of lncRNA-Safe in the AMI group was obviously higher than that in the control group(t=15.736,P<0.01),and the level of lncRNA-Safe in the LVR group was higher than that in the non-LVR group(t=5.290,P<0.01).The left ventricular end-systolic volume(LVESV)and left ventricular end diastolic volume(LVEDV)in the LVR group were higher than those in the non-LVR group,while the left ventricular ejection fraction(LVEF)and left ventricular remodeling index(LVRI)were lower than those in the non-LVR group(t=6.363,3.152,5.759,2.187;P<0.05).Pearson analysis showed that lncRNA-Safe was positively correlated with LVESV and LVEDV(r=0.716,0.701;P<0.01),but negatively correlated with LVEF and LVRI(r=-0.552,-0.442;P<0.01).The area under the curve of lncRNA-Safe in diagnosing LVR in AMI patients was 0.765(95%CI=0.667-0.864),the sensitivity was 64.30%,and the specificity was 86.80%.Conclusion lncRNA-Safe is highly expressed in the serum of AMI patients,and its level is correlated with the occurrence of LVR,and can be used as a serum marker for the diagnosis of LVR in AMI patients.
