血清Hcy、D-二聚体水平与高血压患者认知功能障碍的相关性分析

摘要 目的 探究血清同型半胱氨酸(Hcy)、D-二聚体(D-D)水平与高血压患者认知功能障碍的相关性。方法 选择2019年3月至2021年3月在内黄县人民医院接受治疗的85例高血压患者作为研究组,同时从同期在本院体检的健康者中选取40例作为对照组,检测、比较两组受试者血清Hcy、D-D水平。另对研究组患者行蒙特利尔认知评估量表(MoCA)评分,根据评分结果分为认知功能正常组和认知功能障碍组,并采用Logistic回归法及Pearson相关性分析确定血清Hcy、D-D水平变化与高血压患者认知功能障碍的相关性。结果 研究组患者血清Hcy、D-D水平均高于对照组,差异有统计学意义(P<0.05)。认知功能障碍组患者高血压病程长于认知功能正常组,血清Hcy、D-D水平、脑卒中史、家族史、饮酒史、抽烟史占比高于认知功能正常组,血清UA、HDL水平低于认知功能正常组。经Logistic回归分析,长高血压病程、高Hcy、D-D水平、低UA水平及家族史均是影响高血压患者发生认知功能障碍的危险因素;经Pearson分析,血清Hcy、D-D水平变化与MoCA评分均呈负相关,差异有统计学意义(P<0.05)。结论 高Hcy、D-D水平是影响高血压患者发生认知功能障碍的危险因素,可作为有效临床指标早期监测,为预防高血压认知功能障碍的发生提供数据支持。 Objective To explore the correlation between serum homocysteine(Hcy),D-dimer(D-D) levels,and cognitive dysfunction in patients with hypertension.Methods A total of 85 patients with hypertension who received treatment in Neihuang County People's Hospital from March 2019 to March 2021 were selected as the study group and 40 healthy patients who underwent physical examination in Neihuang County People's Hospital during the same period were selected as the control group.Serum Hcy and D-D levels were detected and compared between the two groups.In addition,montreal cognitive assessment scale(MoCA) scores were performed on patients in the study group,and they were divided into a normal cognitive function group and a cognitive dysfunction group according to the score results.Logistic regression and Pearson correlation analysis were used to determine the correlation between changes in serum Hcy and D-D levels and cognitive dysfunction in hypertensive patients.Results The levels of serum Hcy and D-D in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The course of hypertension in the cognitive dysfunction group was longer than that in the normal cognitive function group,the serum Hcy,D-D,stroke history,family history,drinking history,and smoking history were higher than those in the normal cognitive function group,and the serum UA and HDL levels were lower than those in the normal cognitive function group.Logistic regression analysis showed that a long course of hypertension,high Hcy,D-D levels,low UA levels,and family history were risk factors for cognitive dysfunction in patients with hypertension.Pearson analysis showed that the changes in serum Hcy and D-D levels were negatively correlated with MoCA scores,and the differences were statistically significant(P<0.05).Conclusion High Hcy and D-D levels are risk factors for cognitive dysfunction in hypertensive patients,which can be used as effective clinical indicators for early monitoring,and provide data support for preventing the occurrence of hypertensive cognitive dysfunction.
