鼻内镜手术联合鼻喷布地奈德治疗慢性鼻窦炎伴鼻息肉患者的临床疗效

Effect of Endoscopic Sinus Surgery Combined with Nasal spray Budesonide on Chronic Sinusitis with Nasal polyps

目的 评价鼻内镜手术联合布地奈德喷鼻的治疗方案对慢性鼻窦炎(CRS)伴鼻息肉患者的症状及炎症水平的影响。方法 回顾性分析了河南大学第一附属医院在2019年9月至2022年9月期间收治的120例符合本研究条件的CRS患者,根据治疗方案的不同分为两组,对照组(单独内镜手术治疗)58例,实验组(内镜鼻窦手术联合布地奈德喷鼻治疗)62例。记录并比较两组的治疗效果、治疗前后的鼻腔鼻窦结局测试-22(SNOT-22)、血清嗜酸性粒细胞百分比(EOS%)、血清总免疫球蛋白E(IgE)水平以及不良事件发生情况。结果 实验组的治疗有效率(96.77%)明显高于对照组(79.31%),差异有统计学意义(P<0.05)。干预后,两组SNOT-22评分均有所降低,且实验组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者的EOS和IgE均有显著改善,而实验组的改善优于对照组,差异有统计学意义(P<0.05)。治疗后,对照组的不良反应发生率(20.69%)高于实验组(9.68%)高,差异无统计学意义(P>0.05)。结论 内镜鼻窦手术联合布地奈德喷鼻治疗CRS可有效改善患者的临床症状,能够降低患者的炎症水平。

Objective To evaluate the effect of endoscopic surgery combined with budesonide nasal spray treatment on the symptoms and inflammation levels of patients with chronic rhinosinusitis(CRS)with nasal polyps.Methods A retrospective analysis was conducted on 120 CRS patients who met the study criteria and were treated at the First Affiliated Hospital of Henan University from September 2019 to September 2022.They were divided into two groups according to different treatment plans:the control group(endoscopic surgery alone)with 58 cases,and the experimental group(endoscopic sinus surgery combined with budesonide nasal spray treatment)with 62 cases.The treatment effects,pre-and post-treatment Sino-Nasal Outcome Test-22(SNOT-22),serum eosinophil percentage(EOS%),serum total immunoglobulin E(IgE)levels,and adverse events were recorded and compared between the two groups.Results The effective rate of treatment in the experimental group(96.77%)was significantly higher than that in the control group(79.31%),with a statistically significant difference(P<0.05).After intervention,the SNOT-22 scores in both groups decreased,and the score in the experimental group was lower than that in the control group,with a statistically significant difference(P<0.05).After treatment,both groups showed significant improvements in EOS and IgE,and the improvement in the experimental group was better than that in the control group,with a statistically significant difference(P<0.05).After treatment,the incidence of adverse reactions in the control group(20.69%)was higher than that in the experimental group(9.68%),but the difference was not statistically significant(P>0.05).Conclusion Endoscopic sinus surgery combined with budesonide nasal spray treatment for CRS can effectively improve patients’clinical symptoms and reduce their inflammation levels.
