摘要
目的 探讨评判性思维联合SBAR标准沟通模式在发热门诊中的应用效果。方法 选取2020年2月至2021年6月郑州市第七人民医院发热门诊患者108例进行回顾性分析,根据选取时间进行分组,2020年2月至2020年9月使用基础护理(对照组54例),2020年10月至2021年6月使用SBAR护理(研究组54例),对照组采取常规护理管理,研究组在对照组基础上采取评判性思维联合SBAR标准沟通模式。统计两组护理工作质量、干预前后评判性思维能力(CTDI-CV)、护理满意度、不良事件发生率以及生活质量。结果 研究组健康宣教、操作技能、护理文书、病情掌握能力评分高于对照组,差异有统计学意义(P<0.05);干预后两组CTDI-CV评分较干预前增高,且研究组高于对照组,差异有统计学意义(P<0.05);研究组护理满意度(94.44%)高于对照组(81.48%),差异有统计学意义(P<0.05);两组不良事件发生率比较,研究组不良事件发生率(3.70%)低于对照组(11.11%),差异有统计学意义(P<0.05)。干预后,研究组精力、生理机能等SF-36评分高于对照组,差异有统计学意义(P<0.05)。结论 采取评判性思维联合SBAR标准沟通模式可提升发热门诊护理人员护理工作质量及评判性思维能力,患者对护理工作满意度较高,提升其生活质量。
Objective Investigate the application effectiveness of critical thinking combined with the Situation,Background,Assessment,and Recommendation(SBAR) communication model in a fever outpatient setting.Methods A retrospective analysis was conducted on 108 patients from the Fever Outpatient Department of the Seventh People's Hospital of Zhengzhou City from February 2020 to June 2021.The patients were divided into two groups based on the selected timeframe:the control group(54cases) received basic nursing care from February 2020 to September 2020,while the study group(54 cases) received SBAR nursing care from October 2020 to June 2021.The control group received routine nursing management,while the study group implemented critical thinking combined with the SBAR communication model on top of routine care.The quality of nursing work,critical thinking ability(CTDI-CV) before and after intervention,nursing satisfaction,adverse event occurrence rate,and quality of life were statistically analyzed for both groups.Results The scores of health education,operational skills,nursing documentation,and disease understanding in the intervention group were higher than those in the control group,with statistically significant differences(P<0.05).The CTDI-CV scores of both groups increased after the intervention,and the intervention group had higher scores than the control group,with statistically significant differences(P<0.05).The nursing satisfaction rate in the intervention group(94.44%) was higher than that in the control group(81.48%),with statistically significant differences(P<0.05).When comparing the incidence of adverse events between the two groups,the rate in the intervention group(3.70%)was lower than that in the control group(11.11%),with statistically significant differences(P<0.05).After the intervention,the scores of energy,physical functioning,and other aspects in the SF-36 assessment were higher in the intervention group than in the control group,with statistically significant differences(P<0.05).Conclusion Adopting critical thinking combined with the Situation,Background,Assessment,and Recommendation(SBAR) communication model can improve the quality of care and critical thinking ability of healthcare personnel in the fever outpatient department.Patients have a higher level of satisfaction with nursing work,leading to an improvement in their overall quality of life.
