集束化护理对长期住院的留置导尿管老年患者的应用效果观察

Observation on the Effect of Cluster Nursing on Elderly Patients with Indwelling Catheter in Long-term Hospital

目的研究集束化护理对长期住院的留置导尿管老年患者的应用效果。方法以福建医科大学附属协和医院2018—2021年接收的60例长期住院且因各类原因需要留置导尿管的老年患者为对象,根据患者进院就诊的时间顺序将其分为普通组和研究组。普通组接受常规的护理模式,研究组接受集束化护理模式。试验开展12个月后,比较不同护理模式对长期住院老年患者尿路感染、非尿路感染并发症的差异、非计划拔管率和留置导尿管时间的影响,以及老年患者对两种不同的护理模式满意度的差异。结果经过12个月的不同模式的护理,普通组的尿路感染率、非计划拔管率和留置导尿管时间超过使用集束化护理模式的研究组,P<0.05。普通组非尿路感染率包括漏尿率、血尿、意外移位、皮肤创伤、疼痛或不适高于使用集束化护理模式的研究组,P<0.05。研究组患者对其所接受的集束化护理模式的满意人数明显超过普通组,总满意度比普通组更高,P<0.05。结论集束化护理模式有利于预防长期住院的留置导尿管老年患者的尿路感染率,改善其漏尿情况和因各种原因导致需要非计划拔管的概率,且更易被患者接受,可以运用到长期住院的留置导尿管老年患者的护理中。

Objective To study the application effect of cluster nursing on long-term hospitalized elderly patients with indwelling catheter.Methods 60 elderly patients who had been hospitalized for a long time in Fujian Medical University Union Hospital from 2018 to 2021 and needed indwelling catheters for various reasons were separated into common group and research group.The common group received traditional nursing mode and the research group received cluster intervention nursing mode.12 months after the experiment was carried out,compared the effects of different nursing modes on urinary tract infection and,non-urinary tract infection rate,unplanned extubation and retention time of catheter of long-term hospitalized elderly patients,as well as the divergences in the satisfaction of elderly patients with two different nursing modes.Results After 12 months of different modes of nursing,the urinary tract infection rate,unplanned extubation rate and retention time of catheter of the common group was obviously higher than that of the research group using the cluster nursing mode,P<0.05.The non urinary tract infection rate in the common group,including urinary leakage rate,hematuria,accidental displacement,skin trauma,pain or discomfort,was higher than that in the research group using cluster nursing mode,P<0.05.The number of people satisfied with the cluster nursing mode received by the research group was apparently higher than that of the common group,P<0.05.Conclusion The cluster nursing model is conducive to prevent the urinary tract infection rate of long-term hospitalized elderly patients with indwelling catheter.It can improve the leakage of urine and the probability of unplanned extubation due to various reasons and is more easily accepted by patients.It can be applied to the nursing of long-term hospitalized elderly patients with indwelling catheter.
