摘要
目的:分析阴道微生物与子宫颈人乳头状瘤病毒(HPV)感染及子宫颈病变的关系,为临床诊治提供参考依据。方法:采取回顾性研究法,选取2021年7月至2022年6月本院诊治的80例子宫颈HPV感染患者,纳入HPV感染组;选取本科室同时期诊治的80例子宫颈病变患者,纳入病变组;另选取本院同时期行常规妇科检查的80例志愿者,纳入对照组。三组均行阴道微生物检验,包括形态学(菌群密集度、菌群多样性、乳酸杆菌比例)、功能学(酸碱度、过氧化氢、唾液酸苷酶、白细胞酯酶)以及阴道病菌感染率(细菌性阴道炎、外阴阴道假丝酵母菌病、滴虫性阴道炎),分析三组的阴道微生物检验结果。结果:病变组菌群密集度、菌群多样性、乳酸杆菌比例的异常检出率高于HPV感染组,P<0.05;HPV感染组、病变组菌群密集度、菌群多样性、乳酸杆菌比例的异常检出率高于对照组,P<0.05。病变组酸碱度、过氧化氢、唾液酸苷酶、白细胞酯酶的阳性检出率高于HPV感染组,P<0.05;HPV感染组、病变组酸碱度、过氧化氢、唾液酸苷酶、白细胞酯酶的阳性检出率高于对照组,P<0.05。HPV感染组与病变组细菌性阴道炎、外阴阴道假丝酵母菌病、滴虫性阴道炎的发生率相近,P>0.05;HPV感染组与病变组细菌性阴道炎、外阴阴道假丝酵母菌病、滴虫性阴道炎的发生率高于对照组,P<0.05。结论:阴道微生物与子宫颈HPV感染及子宫颈病变之间存在一定的相关性,通过对阴道微生物的检测,能够为子宫颈HPV感染及子宫颈病变的诊断提供参考依据。
Objective:To analyze the relationship between vaginal microbes and cervical human papillomavirus(HPV)infection and cervical lesions,and to provide reference for clinical diagnosis and treatment.Methods:A retrospective study method was conducted to select 80 patients with cervical HPV infection diagnosed who were treated in our hospital from July 2021 to June 2022,and included in the HPV infection group;80 patients with cervical lesions diagnosed and treated by our department during the same period were selected and included in the lesions Group;another 80 volunteers who underwent routine gynecological examinations in our hospital during the same period were selected and included in the control group.Vaginal microbiological tests were performed in all three groups,including morphological(bacterial density,flora diversity,proportion of lactobacilli),functional(pH,hydrogen peroxide,sialidase,leukocyte esterase)and vaginal bacterial infection rate(bacterial vaginitis,vulvovaginal candidiasis,trichomonas vaginitis),the vaginal microbiological test results of the three groups were analyzed.Results:The abnormal detection rate of flora density,flora diversity and lactobacillus ratio in the lesion group was higher than that in the HPV infection group,P<0.05;The abnormal detection rate of the proportion was higher than that of the control group,P<0.05.The positive detection rates of pH,hydrogen peroxide,sialidase and leukocyte esterase in the lesion group were higher than those in the HPV infection group,P<0.05;The positive detection rate of enzyme was higher than that of the control group,P<0.05.The incidence rates of bacterial vaginosis,vulvovaginal candidiasis,and trichomonas vaginitis between the HPV infection group and the lesion group were similar,P>0.05;the HPV infection group and the lesion group had bacterial vaginitis,vulvovaginal candida The incidence of trichomoniasis and trichomoniasis vaginitis was higher than that of the control group,P<0.05.Conclusion:There is a certain correlation between vaginal microbes and cervical HPV infection and cervical lesions.The detection of vaginal microbes can provide reference for the diagnosis of cervical HPV infection and cervical lesions.
