摘要
目的探讨NCCT测定2~3cm肾盂结石CT值对一期FURL的疗效评估。方法回顾性分析经FURL治疗的85例结石患者临床资料,按CT值分为A组(<800Hu)30例、B组(800~1000Hu)28例、C组(>1000Hu)27例。对比三组患者手术时间、一期SFR及术后CRP、PCT、SCr、BUN水平。结果C组手术时间长于B组,B组长于A组(P<0.05)。C组一期SFR较A组、B组更低(P<0.017),C组术后1天、3天CRP、PCT水平及术后1月SCr、BUN水平均高于A组、B组(P<0.05);而A组、B组无差异(P>0.05)。结论NCCT测定2~3cm肾盂结石CT值对一期FURL疗效有可靠的预估价值,CT值<1000Hu术时较短,一期SFR较高,且术后炎性指标、肾功能可较快恢复。
Objective To investigate the efficacy evaluation of CT value of renal pelvic calculi(2~3cm)measured by NCCT for primary flexible ureteroscopic lithotripsy(FURL).Methods The clinical data of 85 patients with calculi treated by FURL were retrospectively analyzed.According to the CT value of calculi,group A(<800Hu)30 cases,group B(800~1000Hu)28 cases,group C(>1000Hu)27 cases.The operative time,primary stone-free rate(SFR),CRP,PCT,SCr and BUN levels were compared among the three groups.Results The operation time was longer in group C than that in group B,and group B was longer than that in group A(P<0.05).The primary SFR of group C was lower than that in group A and group B(P<0.017).The levels of CRP and PCT on the 1st and 3rd postoperative days,as well as SCr and BUN on the 1st postoperative month in group C was higher than those in group A and group B(P<0.05),but there was no difference between group A and group B(P>0.05).Conclusions CT value of renal pelvic calculi(2~3cm)measured by NCCT for primary FURL has a reliable predictive value.When the CT value is less than 1000Hu,the operative time is shorter,the primary SFR is higher,and the postoperative inflammatory indicators and renal function can recover quickly.
