维持性血液透析患者高磷血症的危险因素研究

Study on the risk factors of hyperphosphatemia in maintenance hemodialysis patients

目的通过分析山东省枣庄市立医院血液透析中心MHD患者临床资料,探讨高磷血症的相关危险因素。方法采用横断面调查方式,纳入MHD患者共152例,收集其临床资料及实验室检查结果。根据血磷水平将MHD患者分为高磷血症及非高磷血症组,采用Spearman相关性分析和logistic回归分析的方法,比较、分析2组间的检验结果。结果152例患者中,107例(70.4%)患者血磷≥1.78 mmol/L。与非高磷血症组相比,高磷血症组的MHD患者更年轻、前白蛋白、白蛋白、肌酐及尿酸等代谢指标水平更高,尿素清除指数更低(P<0.05)。应用多因素Logistic回归分析高磷血症的独立危险因素,结果提示肌酐的OR值为1.005,尿酸的OR值为1.007。结论高水平肌酐和尿酸是MHD患者高磷血症的独立危险因素。

Objective To explore the risk factors of hyperphosphatemia by analyzing the clinical data of maintenance hemodialysis(MHD)patients in hemodialysis center of Zaozhuang Municipal Hospital,Shangdong Province.Methods A total of 152 MHD patients were enrolled in this cross-sectional study.We collected and analyzed their clinical data and laboratory test results.Patients were divided into hyperphosphatemia group and non-hyperphosphatemia group according to their phosphoremia level.We used Spearman correlation analysis and logistic regression analysis to compare and analyze the test results between the two groups.Results Among 152 MHD patients,107(70.4%)patients whose serium phosphorus level≥1.78 mmol/L.Compared with non-hyperphosphatemia group,patients in hyperphosphatemia group were younger,had higher metabolic index of serum prealbumin,albumin,serum creatinine,uric acid,and lower level of urea clearance index(P<0.05).Multi-factor Logistic regression analyzed independent risk factor of hyperphosphatemia,the results showed that the OR value was 1.005,and uric acid was 1.007.Conclusions High level of creatinine and uric acid are the independent risk factors for hyperphosphatemia in MHD patients.
