血清骨钙素、TP1NP、25(OH)D3水平在绝经后女性T2DM患者并发DKD中的应用价值探讨

摘要 目的 分析血清骨钙素(OC)、总I型前胶原N端前肽(TP1NP)、25-羟维生素D3[25(OH)D3]水平在绝经后女性2型糖尿病(T2DM)并发糖尿病肾病(DKD)患者中的应用价值。方法 收集122例绝经后女性T2DM患者临床资料,其中合并DKD 41例(DKD组),无DKD 81例(对照组),比较两组血清OC、TP1NP、25(OH)D3水平,使用受试者工作特征(ROC)曲线评估血清OC、TP1NP、25(OH)D3对绝经后女性T2DM患者合并DKD的诊断价值;并比较不同尿白蛋白/肌酐比值(UACR)的DKD患者血清OC、TP1NP、25(OH)D3水平差异。结果 DKD组血清OC、TP1NP、25(OH)D3水平均低于对照组(P<0.05)。OC、TP1NP、25(OH)D3对绝经后女性T2DM患者合并DKD均有较高诊断价值(P<0.05),且3项联合诊断价值更高(P<0.05)。绝经后女性DKD患者中,UACR>300mg/g者血清OC、TP1NP、25(OH)D3水平明显低于UACR≤300mg/g者(P<0.05)。结论 血清OC、TP1NP、25(OH)D3水平对评估绝经后女性T2DM患者DKD发生、发展进程有利。 Objective To evaluate the application value of serum osteocalcin(OC),total type 1 procollagen N-terminal propeptide(TP1NP)and 25 hydroxyvitamin D 3[25(OH)D 3]levels on postmenopausal women with type 2 diabetes mellitus(T2DM)complicated with diabetic kidney disease(DKD).Methods Clinical data of 122 postmenopausal women with T2DM were collected for the study,including 41 cases with DKD(DKD group)and 81 cases without DKD(control group).Serum levels of OC,TP1NP and 25(OH)D 3 were compared between the two groups.Receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of serum OC,TP1NP and 25(OH)D 3 on DKD in postmenopausal women with T2DM.Serum levels of OC,TP1NP and 25(OH)D 3 were compared among DKD patients with different values of urinary albumin/creatinine ratio(UACR).Results Levels of serum OC,TP1NP and 25(OH)D 3 in DKD group were lower than those in the control group(P<0.05).OC,TP1NP and 25(OH)D 3 had high diagnostic value on DKD in postmenopausal women with T2DM(P<0.05),and the combined performance of the three items was even higher(P<0.05).Among postmenopausal women with DKD,serum levels of OC,TP1NP and 25(OH)D 3 in patients with UACR>300 mg/g were significantly lower than those in patients with UACR≤300 mg/g(P<0.05).Conclusion Serum levels of OC,TP1NP and 25(OH)D 3 are beneficial to evaluating the occurrence and progression of DKD in postmenopausal women with T2DM.
