抑郁症患者和健康人群血清补体C1q、B因子的比较

摘要 目的 比较抑郁症患者和健康人群血清补体C1q、B因子等指标的差异,探究患者体液免疫相关指标变化的原因及其临床应用。方法 本研究回顾性分析了河北医科大学第一医院2020年7月至2022年9月的抑郁症患者173例作为研究对象,匹配同时期河北医科大学第一医院体检中心体检人员120例作为健康对照组研究对象。对两组人群的一般人口学资料及血清学指标进行比较,血清学指标包括C反应蛋白(CRP)、补体C1q(C1q)、B因子(CBF)、补体3(C3)、补体4(C4)、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白E(IgE)等。通过SPSS 23.0进行数据统计分析,比较抑郁症组与健康对照组的血清生物指标的差异,并采用ROC曲线分析上述指标的临床应用价值。结果 抑郁症患者CRP、C1q、CBF、C4、IgA、IgE水平显著高于健康组(P<0.05),IgG水平显著低于健康组(P<0.05)。抑郁症组女性患者较男性患者的C1q、CBF、IgM、IgG水平均显著升高(P<0.05)。Logistic回归控制混杂因素后分析结果提示性别、年龄、CRP、C1q、IgA、IgG、IgE是抑郁症的影响因素,将上述指标进行Logistic回归建模后,联合预测概率对抑郁症预测的AUC为0.814(P<0.01)。结论 抑郁症患者补体系统和免疫球蛋白水平均发生了改变,补体和免疫球蛋白联合测定可作为诊断抑郁症的潜在生物标志物检测组合。 Objective This research aims to explores the differences of serum complement C1q and factor B in patients with depression compared and healthy people,and to identify potential reasons for the changes in humoral immunity related indicators in patients with depression and their clinical applications.Methods We retrospectively analyzed 173 depression patients as research objects and 120 physical examiners during the same period as the control group,who were selected from the First Hospital of Hebei Medical University from July,2020 to September,2022.The general demographic data and serological indicators of the two groups were compared,including C-reactive protein(CRP),complement C1q,factor B(CBF),complement 3(C3),complement 4(C4),immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM),and immunoglobulin E(IgE).SPSS 23.0 was used for statistical analysis to compare serum biological indicators between the depression group and the healthy control group,and the ROC curve was drawn to evaluate the clinical application value of the above indicators.Results Levels of CRP,C1q,CBF,C4,IgA and IgE in patients with depression were significantly higher than those in the healthy group(P<0.05),and the level of IgG was significantly lower than that in the healthy group(P<0.05).Levels of C1q,CBF,IgM and IgG in female patients with depression were significantly higher than those in male patients(P<0.05).The analysis results after controlling for confounding factors by logistic regression suggested that gender,age,CRP,C1q,IgA,IgG and IgE were the influencing factors of depression.With the above indexes modeled by logistic regression,AUC of the combined prediction probability for depression was 0.814(P<0.01).Conclusion Levels of complement system and immunoglobulin in patients with depression can change,and the combined determination of complement and immunoglobulin could be used as a potential biomarker for the diagnosis of depression.
