个体化定制3D打印导板用于全髋关节置换术中的效果研究 被引量:1

摘要 目的:探讨基于三维(3D)打印的个性化截骨导板在全髋关节置换术(THA)中的应用效果。方法:选取医院收治的36例患有髋关节疾病且须行THA的患者,将个性化3D打印导板辅助截骨的患者纳入观察组(20例),传统方法截骨患者纳入对照组(16例),比较两组手术效果和临床疗效。结果:观察组患者手术用时、术中失血量和术后HGB低于对照组,差异均有统计学意义(t=7.548,t=6.447,t=5.175;P<0.05)。观察组患者髋关节修复效果的优良率显著高于对照组,差异有统计学意义(x^(2)=4.410,P<0.05);两组患者髋关节功能恢复情况及术后并发症发生率比较,差异均有统计学意义(x^(2)=3.889,x^(2)=5.400;P<0.05)。结论:基于3D打印技术的个体化导板有较高的截骨精确性和判定假体大小的准确性,用于THA手术中有利于节省手术时间,减少出血量,改善关节功能。 Objective:To explore the application effect of individualized osteotomy guide plate that was printed by three dimensional(3D)technique in total hip arthroplasty(THA).Methods:A total of 36 patients with hip disease who should undergo THA in hospital were selected.The 20 cases who used individualized 3D-printed guide plate to assist osteotomy were divided into the study group,and the 16 cases who used conventional method to conduct osteotomy were divided into the control group.The surgical effects and clinically curative effects of two groups were compared.Results:The operation time,the volume of intraoperative blood loss and the postoperative hemoglobin(HGB)in the study group were significantly lower than those in the control group,and the differences of them were statistically significant(t=7.548,t=6.447,t=5.175,P<0.05),respectively.The excellent rate of the repair effect of the hip joint of patients in study group was significantly higher than that of control group(x^(2)=4.410,P<0.05).There were statistically significant differences in the recovery of the function of hip joint and the complications after surgery between the study group and the control group(x^(2)=3.889,x^(2)=5.400,P<0.05),respectively.Conclusion:The individualized guide plate based on 3D printing technique has higher osteotomy precision,as well as higher accuracy of determining the size of the prosthesis,which is profit to save the operation time,and reduce bleeding and improve the function of joint when it is used in THA.
