腹腔镜结直肠癌根治术患者血清CXCL5联合CYFRA21-1对术后复发转移的预测价值

The predictive value of serum CXCL5 combined with CYFRA21-1 for postoperative recurrence and metastasis in patients undergoing laparoscopic radical resection of colorectal cancer

目的探讨腹腔镜结直肠癌根治术患者血清CXCL5联合CYFRA21-1对术后复发转移的预测价值。方法选取淮安市第一人民医院2017年9月至2022年9月收治的78例接受腹腔镜结直肠癌根治术后复发转移的患者为实验组,同时选取同时间在该院接受腹腔镜结直肠癌根治术后未复发转移来院复查患者78例为对照组,并对患者的病历资料进行回顾性分析。结果实验组在入院第1、3、5、7天的血清CXCL5和CYFRA21-1的水平均呈现上升趋势,高于对照组,且差异有统计学意义(P<0.05);把CXCL5、CYFRA21-1作为自变量进行Logistic回归分析,分析显示CXCL5、CYFRA21-1均是影响腹腔镜结直肠癌根治术后复发转移的因素(P<0.05);单独CXCL5、单独CYFRA21-1和两者联合预测腹腔镜结直肠癌根治术术后复发转移的灵敏度分别为84.2%、84.2%、89.5%;特异度为72.7%、80.3%、93.9%;AUC为0.802、0.821、0.897(P<0.01)。结论CXCL5联合CYFRA21-1预测腹腔镜结直肠癌根治术术后复发转移的灵敏度和特异度均较好,可以在临床上推广使用。

Objective To explore the predictive value of serum CXCL5 combined with CYFRA21-1 in postoperative recurrence and metastasis in patients undergoing laparoscopic radical resection of colorectal cancer.Methods 78 patients from September 2017 to September 2022 at Huai'an First People's Hospital,who underwent laparoscopic radical surgery for colorectal cancer with recurrence and metastasis,were selected as the experimental group.At the same time,78 patients who underwent laparoscopic radical surgery for colorectal cancer without recurrence and metastasis were selected as the control group for re-examination at Huai'an First People's Hospital during the same period.The patient's medical records were retrospectively analyzed.Results The levels of serum CXCL5 and CYFRA21-1 in the experimental group showed an upward trend on the 1st,3rd,5th,and 7th day of admission,and were higher than those in the control group,with statistically significant differences(P<0.05);Using CXCL5 and CYFRA21-1 as independent variables for Logistic Regression Analysis,it was found that both CXCL5 and CYFRA21-1 were influential factors in postoperative recurrence and metastasis of laparoscopic colorectal cancer(P<0.05);The sensitivities of CXCL5 alone,CYFRA21-1 alone,and their combination in predicting postoperative recurrence and metastasis of laparoscopic colorectal cancer radical surgery were 84.2%,84.2%,and 89.5%,respectively;The specificity was 72.7%,80.3%,and 93.9%;AUC is 0.802,0.821,and 0.897(P<0.05).Conclusions CXCL5 combined with CYFRA21-1 has good sensitivity and specificity in predicting postoperative recurrence and metastasis of laparoscopic colorectal cancer and can be widely used in clinical practice.
