聚醚醚酮材料内外排锚钉治疗急性髌骨下极骨折的效果观察

Observation of effect of double-rows technique with polyetheretherketone anchors fixation in the treatment of acute fracture of patellar lower pole

目的观察聚醚醚酮(PEEK)材料内外排锚钉治疗髌骨下极骨折的临床效果。方法抽取2020年7月至2022年6月常熟市第一人民医院骨科收治的急性髌骨下极骨折患者9例,采用PEEK材料内外排锚钉固定治疗骨折,术后进行早期康复锻炼,术后1、3、6个月随访患者膝关节功能,采用Bostman评分体系及膝关节活动度(ROM)评价髌骨骨折术后疗效,并记录手术相关并发症发生情况。结果所有患者均得到完整随访,随访时间6个月。术后1、3、6个月,患者的膝关节功能Bostman评分分别是(21.89±1.83)、(27.33±2.69)、(29.56±1.01)分;术后6个月时Bostman评分情况:优秀8例,良好1例,优良比例为9/9,膝关节屈曲角度为135.9°±3.26°,伸直角度为-1.11°±2.52°。所有患者均未出现手术切口感染、深静脉血栓等相关并发症。结论应用PEEK材料内外排锚钉治疗急性髌骨下极骨折临床效果优良,而且可以行早期膝关节功能锻炼,无需行二次手术取出内植物,是临床治疗急性髌骨下极骨折的可靠方法。

Objective To investigate the clinical effects of double-rows technique with polyetheretherketone(PEEK)anchors fixation in the treatment of acute fracture of patellar lower pole.Methods Nine patients with acute fracture of patellar lower pole treated in Changshu No.1 People’s Hospital from July 2020 to June 2022 were selected.They were treated by double-rows technique with PEEK anchors fixation and performed early-stage rehabilitation training after surgery.Patients were followed up 1,3 and 6 months after surgery to record knee function.The treatment effect after surgery for fracture of patellar lower pole was evaluated by Bostman knee joint scoring system and range of motion(ROM)of knee joint.The complications were recorded after surgery.Results All the patients were followed up for 6 months exactly.The 1-,3-and 6-month postoperative Bostman score were 21.89±1.83,27.33±2.69 and 29.56±1.01,respectively.By the time of 6 month after surgery,8 cases were excellent,and 1 case were good evaluated by Bostman score,with the excellent and good ratio of 9/9.The range of motion was flexion for 135.9°±3.26°,and-1.11°±2.52°for extension.None of the patients had complications,for example,surgical incision infection and deep vein thrombosis.Conclusions Double-rows technique with PEEK anchors fixation,in the treatment of acute fracture of patellar lower pole,has excellent clinical effects,by which patients can perform knee function exercise early without the need for secondary surgery to remove implants.It is a reliable treatment method for acute fracture of patellar lower pole.
