高频肌骨超声在踝关节外侧副韧带损伤诊断中的应用价值 被引量:1

Application value of high-frequency musculoskeletal ultrasound in the diagnosis of lateral collateral ligament injury of ankle joint

摘要 目的分析高频肌骨超声在踝关节外侧副韧带损伤诊断中的应用价值。方法抽取2020年2月至2022年1月平煤神马医疗集团总医院收治的疑似踝关节外侧副韧带损伤患者96例,均行高频肌骨超声和磁共振成像(MRI)检查,以手术探查或修补结果为"金标准",统计高频肌骨超声与MRI的诊断结果和诊断效能,分析其预测值。结果96例疑似踝关节外侧副韧带损伤患者,经手术探查或修补结果确诊阳性53例,阴性43例;高频肌骨超声诊断出真阳性46例,真阴性41例;MRI诊断出真阳性49例,真阴性42例;高频肌骨超声诊断的灵敏度(86.79%、46/53)、特异度(95.35%、41/43)、准确度(90.63%、87/96)、漏诊率(13.21%、7/53)、误诊率(4.65%、2/43)与MRI诊断(92.45%、49/53;97.67%、42/43;94.79%,91/96;7.55%,4/53;2.33%,1/43)比较差异未见统计学意义(P>0.05);高频肌骨超声诊断阳性预测值(95.83%、46/48)、阴性预测值(85.42%、41/48)与MRI诊断(98.00%、49/50;91.30%、42/46)比较差异未见统计学意义(P>0.05)。结论高频肌骨超声应用于踝关节外侧副韧带损伤的诊断具有较高的灵敏度、特异度和准确度,可减少漏诊和误诊情况的发生,为临床采取针对性治疗提供参考依据。 Objective To analyze the application value of high-frequency musculoskeletal ultrasound in the diagnosis of lateral collateral ligament injury of ankle joint.Methods A total of 96 patients suspected of lateral collateral ligament injury of ankle joint who were treated in General Hospital of Pingmei Shenma Group from February 2020 to January 2022 were selected,and they all underwent high-frequency musculoskeletal ultrasound and magnetic resonance imaging(MRI)examinations.With the results of surgical exploration or repair as the gold standard,the diagnostic results and diagnostic efficacy of high-frequency musculoskeletal ultrasound and MRI were analyzed,and the predictive values of them were analyzed.Results Among the 96 patients suspected of lateral collateral ligament injury of ankle joint,53 cases were positive,and 43 cases were negative according to surgical exploration and repair.High-frequency musculoskeletal ultrasound identified 46 cases true positive and 41 cases true negative;MRI identified 49 cases true positive and 42 cases true negative.There was no significant difference in sensitivity,specificity,accuracy,missed diagnosis rate and misdiagnosis rate between high-frequency musculoskeletal ultrasound diagnosis(86.79%,46/53;95.35%,41/43;90.63%,87/96;13.21%,7/53;4.65%,2/43)and MRI diagnosis(92.45%,49/53;97.67%,42/43;94.79%,91/96;7.55%,4/53;2.33%,1/43),P>0.05.The positive predictive value(95.83%,46/48)and negative predictive value(85.42%,41/48)of high-frequency musculoskeletal ultrasound diagnosis were significantly different from those of MRI diagnosis(98.00%,49/50;91.30%,42/46),P>0.05.Conclusions The application of high-frequency musculoskeletal ultrasound in the diagnosis of lateral collateral ligament injury of ankle joint has a high diagnostic sensitivity,specificity and accuracy,which can reduce missed diagnosis and misdiagnosis,and provide reference for clinical targeted treatment.
