曲美他嗪联合常规治疗对急性心肌炎心功能不全及心律失常患者的疗效研究

Therapeutic effect of trimetazidine combined with conventional treatment on acute myocarditis complicated by cardiac insufficiency and arrhythmia

目的分析曲美他嗪联合常规治疗急性心肌炎心功能不全及心律失常的临床效果。方法抽取2019年6月至2022年6月平顶山市第一人民医院收治的急性心肌炎心功能不全及心律失常患者120例,按随机数字表法分为观察组与对照组,每组60例。对照组采用常规治疗,观察组在对照组的基础上采用曲美他嗪治疗。比较两组疗效和治疗前后心功能[左心室舒张末期内径(LVEDD)、左室射血分数(LVEF)]、B型脑钠肽(BNP)水平、N末端B型脑钠肽(NT-proBNP)水平、心肌酶指标[肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)]。结果观察组总有效率(93.33%,56/60)高于对照组(75.00%,45/60),P<0.05。治疗后,观察组LVEDD低于对照组,LVEF水平高于对照组(P<0.05)。治疗后,观察组BNP、NT-proBNP水平低于对照组(P<0.05)。治疗后,观察组CK、CK-MB、LDH、AST水平低于对照组(P<0.05)。结论曲美他嗪联合常规治疗急性心肌炎心功能不全及心律失常患者效果显著,可改善心功能,减轻心肌损伤。

Objective To analyze the clinical effect of trimetazidine combined with conventional treatment on acute myocarditis complicated by cardiac insufficiency and arrhythmia.Methods A total of 120 patients with acute myocarditis complicated by cardiac insufficiency and arrhythmia treated in Pingdingshan First People’s Hospital from June 2019 to June 2022 were selected,and they were divided into observation group and control group according to random number table method,with 60 cases in each group.The control group received conventional treatment,and the observation group received trimetazidine based on the treatment of the control group.The treatment efficacy of the two groups were compared.The cardiac function assessed by left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF),B-type brain natriuretic peptide(BNP)level,N-terminal B-type brain natriuretic peptide(NT-proBNP)level,myocardial enzyme indexes,including creatine kinase(CK),creatine kinase isoenzyme(CK-MB),lactate dehydrogenase(LDH)and aspartate aminotransferase(AST),were compared between the two groups before and after treatment.Results The total effective rate of the observation group(93.33%,56/60)was higher than that of the control group(75.00%,45/60),P<0.05.After treatment,the LVEDD of the observation group was lower than that of the control group,and the LVEF level was higher than that of the control group(P<0.05).After treatment,the levels of BNP and NT-proBNP in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of CK,CK-MB,LDH and AST in the observation group were lower than those in the control group(P<0.05).Conclusions Trimetazidine combined with conventional treatment has a significant effect on acute myocarditis complicated by cardiac insufficiency and arrhythmia.What’s more,it can improve cardiac function and reduce myocardial injury.
