上海市某医院产科住院患者费用及其影响因素分析

Analysis on the Costs of Obstetric Inpatients of a Hospital in Shanghai and Its Influencing factors

目的探讨产科住院患者费用构成及其影响因素,为产科住院费用的控制和政策制定提供一定的参考借鉴。方法选取上海市某医院产科2021年1月1日至12月31日出院1000例住院产妇的住院总费用及构成情况,采用单因素方差分析和多元逐步回归分析法对住院费用的影响因素进行统计分析。结果1000例产妇的平均住院费用为12097.45元,影响住院费用的影响因素包括分娩方式的选择、患者年龄、婚姻状况、付款方式、是否抢救患者、是否具有产科并发症等,其中分娩方式为影响产科住院费用的最大因素,剖宫产方式的产妇平均住院费用为13209.32元,显著高于顺产或者其他助产方式的住院费用,具有产科并发症的产妇住院费用要高于无并发症的产妇住院费用,均P<0.05。结论分娩方式和产科并发症是影响产妇住院费用的主要因素,控制剖宫产发生率、积极减少产科并发症的发生,控制医疗费用的支出,进一步提高医院的管理水平和工作效率。

Objectives To explore the composition of obstetric inpatient costs and its influencing factors,so as to provide reference for the effective control of obstetric discharge costs.Methods The total hospitalization expenses and composition of 1000 hospitalized patients discharged from a hospital in Shanghai from January 1st to December 31st,2021 were selected,and the influencing factors of hospitalization expenses were statistically analyzed by one-way ANOVA and multiple stepwise regression analysis.Results The average hospitalization expense of 1000 patients was 12097.45 yuan.The influencing factors of hospitalization expense included the choice of delivery mode,patient age,marital status,payment mode,whether to rescue the patient,and whether there were obstetric complications.Among them,the delivery mode was the largest factor affecting obstetric hospitalization expense,and the average hospitalization expense of patients with cesarean section was 13209.32 yuan,It was significantly higher than the hospitalization expenses of natural delivery or other midwifery methods,both P<0.05.Conclusions The mode of delivery and obstetric complications were the main factors affecting the cost of maternal discharge.Controlling the incidence of cesarean section and actively reducing the incidence of obstetric complications could not only reduce the pain of maternal delivery,but also effectively reduce the cost of maternal hospitalization.
