摘要
目的:探讨紫杉醇药物涂层球囊(DCB)对比普通球囊治疗膝下动脉硬化闭塞症的疗效。方法:选取220例膝下动脉硬化闭塞症患者为研究对象,按照接受治疗方案不同分为观察组和对照组,每组各110例。观察组采用DCB治疗;对照组采用普通球囊(POBA)治疗。观察并比较两组患者治疗后临床疗效、影像学指标、病情严重程度、NLR和CRP值、并发症等指标。结果:治疗后,观察组总有效率为98.18%,高于对照组的91.82%(P<0.05);观察组下肢动脉靶血管最小直径(MLD)大于对照组(P<0.05);靶血管晚期管腔丢失情况(LLL)小于对照组(P<0.05);观察组Rutherford分级情况优于对照组(P<0.05);观察组中性粒细胞/淋巴细胞比值(NLR)、CRP小于对照组(P<0.05);观察组并发症总发生率与对照组比较,差异无统计学意义(P>0.05)。结论:紫杉醇药物涂层球囊治疗膝下动脉硬化闭塞症能够提高临床疗效,改善影像学指标情况,减轻炎症反应程度,安全性较高。
Objective:To investigate the effect of paclitaxel drug-coated balloon(DCB)compared with conventional balloon in the treatment of subpatellar arteriosclerosis oblusion.Methods:A total of 220 patients with subknee arteriosclerosis obliterans were selected and divided into two groups according to different treatment plans,with 110 cases in each group.The observation group were treated with DCB and the control group were treated with POBA.The clinical efficacy,imaging indexes,severity of disease,lymphocyte ratio(NLR)and C-reactive protein(CRP)values,and complications were observed and compared between the two groups after treatment.Results:After treatment,the total effective rate of observation group was 98.18%,which was higher 91.82% in the control group(P<0.05).The minimal lumen diameter(MLD)of lower limb artery target vessels in the observation group was greater than that in the control group,the late lumen loss(LLL)of target vessels was lower than that of the control group(P<0.05).The Rutherford grading of the observation group was better than that of the control group(P<0.05).Neutrophil/lymphocyte ratio(NLR)and CRP in the observation group were lower than those in the control group(P<0.05).There was no difference in the total complication rate of observation group compared with control group(P>0.05).Conclusion:Paclitaxel coated balloon in the treatment of subpatellar arteriosclerosis obliterans can improve the clinical efficacy,improve the imaging indicators,reduce the degree of inflammation,high safety.
