益气消肾颗粒治疗早期糖尿病肾脏病临床疗效及对血清CRP、尿NAG水平的影响

Clinical efficacy of Yiqi Xiaoshen granules in the treatment of early diabetic kidney disease and its influence on the levels of serum CRP and urinary NAG

目的 探讨益气消肾颗粒治疗早期糖尿病肾脏病临床疗效及对血清C反应蛋白(CRP)、尿N-乙酰-β-D氨基葡萄糖苷酶(NAG)的影响。方法 按照随机数字表法将72例早期糖尿病肾脏病患者分成对照组及治疗组各36例。对照组予西医常规治疗,治疗组在对照组基础上加用益气消肾颗粒口服治疗,2组均治疗12周。结果 治疗组治疗后的中医证候积分、尿蛋白水平[尿微量白蛋白/尿肌酐(UACR)、24小时尿蛋白(24 h Upro)]、血糖水平[空腹血糖(FPG)、餐后2小时血糖(2 h PG)、糖化血红蛋白(Hb A1c)]、CRP、NAG水平明显低于对照组(P <0.05);治疗组、对照组总有效率分别为87.10%、73.33%,差异具有统计学意义(P <0.05);2组均未出现与本研究药物相关的不良反应,估算肾小球滤过率(e GFR)控制平稳。结论 益气消肾颗粒可以有效改善早期糖尿病肾脏病患者的临床症状,减少尿蛋白,改善糖代谢及体内炎性状态,有助于延缓病情的进展。

Objective To investigate the clinical efficacy of Yiqi Xiaoshen granules in the treatment of early diabetic kidney disease and its influence on the levels of serum C-reactive protein(CRP)and urine N-acetyl-β-D glucosaminidase(NAG).Methods According to the random number table method,72 patients with early diabetic kidney disease were divided into a control group and a treatment group,with 36 cases in each group.The control group was treated with conventional western medicine,while the treatment group was treated with Yiqi Xiaoshen granules on the basis of the control group.Both groups were treated for 12 weeks.Results After treatment,the TCM syndrome scores,urine protein levels[urine microalbumin/urine creatinine(UACR),24-hour urine protein(24 h Upro)],blood glucose levels[fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2 h PG),glycosylated hemoglobin(HbA1c)],C-reactive protein(CRP)and NAG levels in the treatment group were significantly lower than those in the control group(P<0.05);The total effective rate of the treatment group and the control group were 87.10%and 73.33%,respectively,with a statistically significant difference(P<0.05).No adverse reactions related to the drugs in this study occurred in both groups,and the estimated glomerular filtration rate(eGFR)was controlled smoothly.Conclusion Yiqi Xiaoshen granules can effectively improve the clinical symptoms of early diabetic kidney disease,reduce the levels of urinary protein,improve the glucose metabolism and the inflammatory state of the body and help to delay the progression of the disease.
