鸦胆子油乳注射液联合化疗治疗非小细胞肺癌的效果及对患者免疫功能的影响

Therapeutic effect of brucea javanica oil emulsion injection combined with chemotherapy on non-small cell lung cancer and influence on immune function

目的探讨鸦胆子油乳注射液联合化疗治疗非小细胞肺癌(NSCLC)的效果及对患者免疫功能的影响。方法抽取2020年3月至2022年3月于郑州市第七人民医院进行化疗治疗的NSCLC患者72例,采用随机数字表法将其分为注射液组和化疗组,每组36例。化疗组单纯给予化疗治疗,注射液组在化疗组基础上给予鸦胆子油乳注射液治疗。比较两组临床疗效及心脏毒性、免疫功能、生活质量。结果治疗3个疗程后,注射液组总有效率(83.33%,30/36)高于化疗组(61.11%,22/36),P<0.05。治疗后,两组心率(HR)均低于治疗前(P均<0.05),N末端脑钠肽前体(NT-proBNP)和心肌肌钙蛋白I(cTnI)水平高于治疗前(P<0.05),且两组间HR、NT-proBNP、cTnI比较差异有统计学意义(P<0.05)。治疗后,两组CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)比值均低于治疗前(P均<0.05),CD8^(+)水平高于治疗前(P<0.05),且两组间各免疫功能指标比较差异均有统计学意义(P均<0.05)。治疗后,两组生活质量评分均高于治疗前(P均<0.05),且注射液组生理状况和功能状况评分高于化疗组(P<0.05)。结论鸦胆子油乳注射液联合化疗治疗NSCLC的临床疗效显著,可有效降低患者的心脏毒性反应,提高其免疫功能和生活质量。

Objective To investigate the therapeutic effect of brucea javanica oil emulsion injection combined with chemotherapy on non-small cell lung cancer(NSCLC)and its influence on immune function.Methods Seventy-two patients with NSCLC who were treated by chemotherapy in the Seventh People’s Hospital of Zhengzhou from March 2020 to March 2022 were selected,and they were divided into injection group and chemotherapy group using a random number table method,with 36 patients in each group.The chemotherapy group was treated by chemotherapy alone,and the injection group was additionally given brucea javanica oil emulsion injection based on the treatment of the chemotherapy group.The clinical efficacy,cardiotoxicity,immune function and quality of life were compared between the two groups.Results After 3 courses of treatment,the total effective rate in the injection group(83.33%,30/36)was higher than that in the chemotherapy group(61.11%,22/36),P<0.05.After treatment,the heart rate(HR)of the two groups were lower than those before treatment(both P<0.05),while the levels of N-terminal pro-brain natriuretic peptide(NT-proBNP)and cardiac troponin I(cTnI)were higher than that those before treatment(P<0.05).There was a statistically significant difference between the two groups in HR,NT-proBNP and cTnI after treatment(P<0.05).After treatment,the levels of cluster of differentiation 3^(+)(CD3^(+)),cluster of differentiation 4^(+)(CD4^(+))and CD4^(+)/cluster of differentiation 8^(+)(CD8^(+))ratio in the two groups were lower than those before treatment(all P<0.05),while the levels of CD8^(+)were higher than those before treatment(P<0.05),and there were significant differences in the above immune function indicators between the two groups(all P<0.05).After treatment,the score of every aspect of quality of life was improved in both groups(all P<0.05),and the scores of physiological status and functional status in the injection group were higher than those in the chemotherapy group(P<0.05).Conclusions Brucea javanica oil emulsion injection combined with chemotherapy has a significant therapeutic effect on NSCLC,and it can effectively reduce the cardiotoxicity and enhance the immune function and quality of life of patients.
