基于帕累托图的口腔黏膜科不合理处方分析

Analysis of irrational prescriptions in oral mucosa department based on Pareto chart

目的 了解口腔黏膜科不合理处方现状,为干预不合理处方提供参考。方法 随机抽取2022年1月~10月10 000张口腔黏膜科处方实施处方点评,并用帕累托图对口腔黏膜科不合理处方类型、不合理使用药品进行统计与分析。结果 不合理处方94张,占点评处方数的0.94%,其中,临床诊断书写不全(占41.49%)、处方书写不规范(占18.09%)和遴选药品不适宜(占12.77%)的累计构成比72.34%,为不合理处方的主要因素。不合理使用药品102例次,其中,制霉素片(占19.61%)、西吡氯铵含片(占18.63%)和维生素B_(1)片(占13.73%)以及其他4种药品的累计构成比77.45%,为不合理使用药品的主要因素。结论 帕累托图可直观反映口腔黏膜科主要不合理处方类型、主要不合理使用药品,医师应重点关注临床诊断书写不全、处方书写不规范和遴选药品不适宜处方,以及制霉素片、西吡氯铵含片和维生素B_(1)片等7种药品的使用合理性。

Objective To investigate the current situation of irrational prescriptions in oral mucosa department so as to provide reference for intervention of irrational prescriptions.Methods 10000 prescriptions of oral mucosa department from January to October 2022 were randomly selected for prescription review,and the types of irrational prescriptions and irrational use of drugs were count and analyzed by Pareto chart.Results There were 94 irrational prescriptions,accounting for 0.94%of the number of prescriptions reviewed.Among them,incomplete clinical diagnosis(41.49%),nonstandard prescription writing(18.09%)and inappropriate drug selection(12.77%)were the main factors of irrational prescriptions,their cumulative composition ratio was 72.34%.There were 102 cases of irrational drug use.Among them,the cumulative composition ratio of nystatin tablets(19.61%),cetylpyridinium chloride buccal tablets(18.63%),vitamin B_(1) tablets(13.73%)and other four drugs was 77.45%,which was the main factor of irrational drug use.Conclusion Pareto chart can directly reflect the main types of irrational prescriptions and irrational drug use in oral mucosa department.Physicians should focus on incomplete clinical diagnosis,nonstandard prescription writing and inappropriate drug selection,as well as the rationality of the use of 7 drugs,including nystatin tablets,cetylpyridinium chloride buccal tablets,vitamin B_(1) tablets,etc.
