甲状腺乳头状癌患者超声及临床特征与BRAF基因检测结果的相关性分析

摘要 目的:探究甲状腺乳头状癌患者超声及临床特征与BRAF基因检测结果的相关性。方法:回顾性选取2022年1月—2023年5月在石家庄市人民医院接受治疗的甲状腺乳头状癌患者60例,对所有患者均进行BRAF基因检测,根据其检测结果(有无BRAF基因突变)将患者分为两组,27例发生BRAF基因突变的患者纳入突变组,33例未发生BRAF基因突变的患者纳入野生组。对两组患者进行超声检查,分析其超声检查结果的异同,并对患者的临床特征进行比较,探究甲状腺乳头状癌患者超声检查结果、临床特征与患者BRAF基因突变的关系。结果:两组间超声征象比较,在峡部距离是否>2 mm、边界是否清晰、内部回声、有无钙化、血流特征是否丰富方面差异存在统计学意义(P<0.05)。突变组患者的FT4、FT3、TSH、TgAb、TPOAb水平高于野生组(P<0.05),在结节数目、肿瘤原发灶情况方面存在统计学差异(P<0.05)。对患者的超声征象、临床特征与患者BRAF基因突变的相关性采用皮尔逊(Pearson)相关性分析,结果可知BRAF基因突变与患者的内部回声、血流特征、FT3、TSH、TgAb、TPOAb间存在强正相关,其r值分别为0.596、0.607、0.746、0.648、0.969、0.872。结论:甲状腺乳头状癌患者BRAF基因突变可能与患者超声征象及临床特征存在一定关联,患者BRAF基因发生突变后,多表现为边界不清晰、内部回声非低回声、血流丰富,且TgAb、TPOAb水平升高。 Objective To explore the correlation between ultrasound and clinical characteristics and BRAF gene test results in patients with papillary thyroid carcinoma.Methods Retrospectively selected 60 patients with papillary thyroid carcinoma treated in Shijiazhuang People's Hospital from January 2022 to May 2023,all patients were tested for BRAF gene.The patients were divided into two groups according to their test results(with or without BRAF mutations),27 patients with BRAF mutations were included in the mutant group,and 33 patients without BRAF mutations were included in the wild group.To analyze the similarities and differences of the ultrasound results,and compare the clinical characteristics to explore the relationship between the ultrasound results and clinical characteristics of the patients with papillary thyroid carcinoma and the BRAF gene mutations.Results By comparing ultrasound signs between the two groups,there were statistical differences in whether the isthmus distance was>2 mm,clear boundary,internal echo,calcification,and rich blood flow characteristics(P<0.05).The levels of FT4,FT3,TSH,TgAb,and TPOAb in the mutant group were higher than those in the wild group(P<0.05),which were statistically different in the number of nodules and tumor primary condition(P<0.05).The correlation between BRAF gene mutations and the results showed that a strong positive correlation between BRAF gene mutations and internal echo,blood flow characteristics,FT3,TSH,TgAb,and TPOAb,with r values of 0.596,0.607,0.746,0.648,0.969,and 0.872,respectively.Conclusion BRAF gene mutations in papillary thyroid carcinoma may have some association with ultrasound signs and clinical characteristics of patients.After BRAF gene mutations,most patients show unclear boundary,non-hypoecho,rich blood flow,and elevated TgAb and TPOAb levels.
