血清同型半胱氨酸、胱抑素c水平与急性脑梗死患者神经功能缺损程度的相关性分析

Correlation Analysis of Serum Homocysteine and Cystatin c Levels with the Degree of Neurological Impairment in Patients with Acute Cerebral Infarction

目的 分析血清同型半胱氨酸(Hcy)、胱抑素C(Cys-C)水平与急性脑梗死(acute cerebral infarction, ACI)患者神经功能缺损程度的相关性。方法 按照随机抽样法选取2020年09月-2022年09月期间90例ACI患者作为研究对象,按照神经功能缺损量表(NIHSS)分成三组(轻度(n=30)、中度(n=30)、重度(n=30)),观察三组患者的基本资料及血清Hcy、Cys-C水平,分析Hcy、Cys-C水平与ACI患者神经功能缺损程度的相关性。结果 三组患者的性别、年龄、BMI、高血压、高脂血症及糖尿病史等基本资料相比,差异均无明显差别(P>0.05);三组血清Hcy、Cys-C水平相比,差异均具有显著性(P<0.05),其中重度组血清Hcy、Cys-C水平均高于中度组Hcy、Cys-C水平,差异具有显著性(tHcy=12.406,tCys-C=8.498,P<0.05),中度组Hcy、Cys-C水平均高于轻度组Hcy、Cys-C水平,差异具有显著性(tHcy=12.309,tCys-C=8.238,P<0.05);血清Hcy、Cys-C水平与ACI患者神经缺损程度呈正相关关系,差异均具有显著性(P<0.05)。结论 血清Hcy、Cys-C水平变化与ACI患者神经功能缺损程度呈正相关关系,即血清中Hcy、Cys-C水平越高,患者的神经功能缺损程度越严重。

Objective To investigate the relationship between serum homocysteine(Hcy)and cystatin C(Cys-C)levels and neurological deficits in patients with acute cerebral infarction(ACI).Methods Ninety ACI patients from September 2020 to September 2022 were selected by random sampling and divided into three groups(mild(n=30),moderate(n=30)and severe(n=30))according to the Neurological Deficit Scale(NIHSS).The basic data and serum Hcy and Cys-C levels of the three groups of patients were observed,and the correlation between the levels of Hcy and Cys-C and the degree of neurological impairment in ACI patients was analyzed.Results There were no significant differences in gender,age,BMI,hypertension,hyperlipidemia and diabetes history among the three groups(P>0.05).There were significant differences in serum Hcy and Cys-C levels among the three groups(P<0.05),and the serum Hcy and Cys-C levels in the severe group were higher than those in the moderate group,with significant differences(tHcy=12.406,tCys-C=8.498,P<0.05).The levels of Hcy and Cys-C in moderate group were higher than those in mild group,and the differences were significant(tHcy=12.309,tCys-C=8.238,P<0.05).Serum Hcy and Cys-C levels were positively correlated with the degree of nerve defect in ACI patients,and the differences were significant(P<0.05).Conclusions Serum Hcy and Cys-C levels are positively correlated with the degree of neurological impairment in ACI patients,that is,the higher the serum Hcy and Cys-C levels,the more serious the degree of neurological impairment.
