家庭医生签约管理对老年2型糖尿病伴轻度认知功能障碍患者认知功能的影响

Influence of family doctor contract management on the cognitive function in elderly type 2 diabetes patients with mild cognitive impairment

目的:分析家庭医生签约管理对老年2型糖尿病伴轻度认知功能障碍患者认知功能的影响。方法:在2020年1月至2020年6月门诊就诊的老年2型糖尿病患者中,采用蒙特利尔认知评估量表(MoCA)筛查出100例轻度认知功能障碍患者。按是否与家庭医生签约分成签约组和对照组。每组各50例。签约组中有男性22例,女性28例,平均年龄(65.94±3.34)岁,对照组中有男性20例,女性30例,平均年龄(65.74±3.12)岁。签约组在对照组基础上进行家庭医生签约管理。评估两组患者管理6个月和12个月后的认知功能改变情况。结果:签约组患者入组12个月后的MoCA总分高于入组时,且高于对照组,差异均有统计学意义(均P<0.05)。两组患者的简易精神状态检查量表(MMSE)评分差异无统计学意义(P>0.05)结论:家庭医生签约管理在一定程度上可以改善老年2型糖尿病伴轻度认知功能障碍患者的认知功能。

Objective:To analyze the influence of family doctor contract management on the cognitive function of the elderly type 2 diabetes patients with mild cognitive impairment.Methods:From January to June 2020,100 elderly patients with mild cognitive impairment were screened out in the elderly type 2 diabetes patients in the outpatient clinic by MoCA scale.They were divided into a signed group and a control group with 50 cases each according to whether they were signed by family doctors or not.The signed group had 22 male cases and 28 female cases,and the average age was(65.94±3.34)years,and the control group had 20 male cases and 30 female cases and the average age(65.74±3.12)years.The signed group was received family doctor contract management on the basis of the control group.The change of cognitive function in the both groups after 6 and 12 months were evaluated.Results:The score of MoCA in the signed group was significantly higher after 12 months than that before,and that in the control group and the difference was statistically significant(P<0.05).There was no significant difference in scores of mini-mental state examination(MMSE)in the patients in both groups(P<0.05).Conclusion:Family doctor contract management can improve the cognitive function of the elderly type 2 diabetes patients with mild cognitive impairment.
