年轻乳腺癌患者的临床病理特征及临床预后分析

Clinicopathological Features and Clinical Prognosis of Young Breast Cancer

目的:分析年轻乳腺癌患者临床病理特征及其预后。方法:选取2019年1月至2022年5月南华大学附属第二医院收治的30例年轻乳腺癌患者(20~40岁)与同期就诊30例中老年乳腺癌患者(45~65岁)作为研究对象,分别设为观察组和对照组,比较两组患者的临床病理特征,并分析临床预后情况。结果:观察组浸润性导管癌、组织分级Ⅲ级、淋巴结转移、人表皮生长因子受体–2(Her–2)阳性者占比均高于对照组;而浸润性小叶癌、组织分级Ⅰ级、雌激素受体(ER)阳性、孕激素受体(PR)阳性者占比均低于对照组,差异均具有统计学意义(P<0.05);两组患者的乳腺癌分子分型情况比较,差异均无统计学意义(P>0.05);经过12个月的随访,观察组患者病死率高于对照组,生存率低于对照组,差异具有统计学意义(P<0.05)。结论:年轻乳腺癌患者具有淋巴结转移情况及Her–2阳性高、组织分级高等临床病理特征,与中老年乳腺癌患者疾病病理特征存在差异,对患者生存率的影响较大,预后不佳。

Objective To analyze the clinicopathological features and prognosis of 30 young patients with breast cancer.Methods Thirty young breast cancer patients(20~40 years old)admitted to the Second Affiliated Hospital of University of South China from January 2019 to May 2022 and 30 middle-aged and elderly patients with breast cancer(45~65 years old)admitted to the Second Hospital of the University of South China during the same period were selected as the study objects,and were set as an observation group and a control group,respectively,to compare the clinicopathological characteristics of the two groups of patients,and the clinical prognosis was analyzed.Results The percentages of invasive ductal carcinoma,tissue gradeⅢ,lymph node metastasis and human epidermal growthfactor receptor-2(Her-2)positive in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The percentages of invasive lobular carcinoma,tissue gradeⅠ,estrogen receptor(ER)positive and progesterone receptor(PR)positive of the observation group was lower than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the molecular classification of breast cancer between the two groups(P>0.05).After 12 months of follow-up,the mortality rate of the observation group was higher than that of the control group,and the survival rate was lower than that of the control group,with statistical significance(P<0.05).Conclusion Young patients with breast cancer have clinicopathological features of lymph node metastasis,high HER-2 positive and high tissue grade,which are different from middle-aged and elderly patients with breast cancer,which may have a greater impact on the survival rate of patients and a poor prognosis.
