单侧双通道内镜技术对腰椎管狭窄患者术后疼痛及腰椎功能的影响

Effects of Unilateral Biportal Endoscopy Technique on Postoperative Pain and Lumbar Function in Patients with Lumbar Spinal Stenosis

目的探讨单侧双通道内镜(UBE)技术对腰椎管狭窄(LSS)患者术后疼痛及腰椎功能的影响。方法回顾性分析2020年6月至2022年6月于医院完成手术治疗的86例LSS患者的临床资料,根据不同术式分为对照组与试验组,每组43例。对照组接受传统椎板间开窗减压术,试验组接受UBE技术减压治疗,均至少随访6个月。比较两组手术疗效、腰腿疼痛程度[视觉模拟评分法(VAS)评分]、腰椎功能[Oswestry功能障碍指数(ODI)]及并发症发生情况。结果两组手术优良率及并发症发生率比较,差异均无统计学意义(P>0.05)。术前及术后6个月,两组VAS评分及ODI比较,差异均无统计学意义(P>0.05);术后1、3个月,试验组VAS评分及ODI均低于对照组,差异均有统计学意义(P<0.05)。结论UBE技术应用于LSS患者,可与传统椎板间开窗减压术获得相似的短期疗效,且更利于减轻患者腰腿疼痛,改善腰椎功能。

Objective The effects of unilateral biportal endoscopy(UBE)technique on postoperative pain and lumbar function in patients with lumbar spinal stenosis(LSS)were explored.Methods The clinical data of 86 patients with LSS who completed operation in the hospital from June 2020 to June 2022 were retrospectively analyzed,and they were divided into a control group and an experimental group according to different operation methods,with 43 cases in each group.The control group received traditional fenestration decompression and the experimental group received UBE decompression.All patients were followed up for at least 6 months.The surgical effects,the degree of lumbar and leg pain[visual analogue scale(VAS)scores],lumbar function[Oswestry disability index(ODI)]and the incidence of complications were compared between the two groups.Results There was no significant difference in the rate of surgical excellence and complication between the two groups(P>0.05).Before surgery and six months after surgery,there was no significant difference in VAS scores and ODI between the two groups(P>0.05).At 1 and 3 months after operation,VAS scores and ODI of the experimental group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The application of UBE technique in LSS patients can achieve similar short-term efficacy as traditional fenestration decompression,and it is more beneficial to reduce patients'back and leg pain and improve lumbar function.
