高流量无创呼吸湿化治疗仪联合吸气肌功能训练对慢性阻塞性肺疾病急性发作期患者的护理效果 被引量:2

摘要 目的探讨高流量无创呼吸湿化治疗仪联合吸气肌功能训练对慢性阻塞性肺疾病急性加重期(AECOPD)患者的护理效果。方法选取2021年3月至2022年9月医院收治的100例AECOPD患者作为研究对象,采用随机数表法分为对照组和观察组,每组50例。对照组在常规干预基础上采用吸气肌功能训练干预,观察组在对照组基础上加用高流量无创呼吸湿化治疗仪干预。比较两组肺功能相关指标[呼吸频率、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、排痰情况(自主排痰率及3 d内总排痰次数)及舒适度[简化舒适状况量表(GCQ)评分]。结果干预前,两组呼吸频率、PaO_(2)、PaCO_(2)、自主排痰率、3 d内总排痰次数及GCQ评分比较,差异均无统计学意义(P>0.05)。干预后,两组呼吸频率、PaO_(2)及PaCO_(2)均优于干预前,且观察组均优于对照组,差异有统计学意义(P<0.05);两组自主排痰率及GCQ评分均高于干预前,且观察组均高于对照组,差异有统计学意义(P<0.05);两组3 d内总排痰次数均少于干预前,且观察组少于对照组,差异有统计学意义(P<0.05)。结论高流量无创呼吸湿化治疗仪联合吸气肌功能训练可显著改善AECOPD患者肺功能,减少痰量并使痰液稀释便于咳出,提升患者舒适度。 Objective To explore the effects of high-flow non-invasive respiratory humidification therapy instrument combined with inspiratory muscle function training nursing on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients.Methods A total of 100 cases of AECOPD patients admitted to the hospital from March 2021 to September 2022 were selected as the study objects,and randomly divided into a control group and an observation group by random number table method,with 50 cases in each group.Based on the routine intervention,the control group was treated with inspiratory muscle function training intervention,and the observation group was treated with high-flow non-invasive respiratory humidification therapy instrument intervention on the basis of the control group.The indexes related to pulmonary function[respiratory rate,arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))],expectoration status(autonomous expectoration rate and total expectoration times within 3 days)and comfortableness[general comfort questionnaire(GCQ)scores]were compared between the two groups.Results Before intervention,there was no statistically significant differences in the respiratory rate,PaO_(2),PaCO_(2),autonomous expectoration rate,total expectoration times within 3 days and GCQ scores of the two groups(P>0.05).After intervention,the respiratory rate,PaO_(2),PaCO_(2) of the two groups were better than those before intervention,and those of the observation group were better than those of the control group,with statistically significant differences(P<0.05);the autonomous expectoration rate and GCQ scores of the two groups were higher than those before intervention,and those of the observation group were higher than those of the control group,with statistically significant differences(P<0.05);the total expectoration times within 3 days of the two groups were less than before intervention,and that of the observation group was less than that of the control group,with statistically significant differences(P<0.05).Conclusion The application of high-flow non-invasive respiratory humidification therapy instrument combined with inspiratory muscle function training in patients with AECOPD can significantly improve their pulmonary function,reduce sputum volume and dilute sputum to facilitate coughing up,and improve patients'comfortableness.
