痔疮术后穴位镇痛治疗1117例效果影响因素分析

Analysis of Factors Influencing of Acupoint Analgesia on 1117 Patients with Hemorrhoids after Operation

目的回顾性分析痔疮术后穴位镇痛治疗效果与影响因素。方法收集福建中医药大学附属人民医院肛肠二科近5年痔疮术后穴位镇痛治疗资料完整者1117例,分析性别、年龄、依从性、吸烟、糖尿病、高血压病、术前镇静镇痛药使用或依赖、麻醉方式、术式、手术时长、术灶位置与数量、初次手术与否、术毕至治疗的时间、治疗时长与频度、取穴数目等因素与疗效的相关性。结果总有效率为73.05%(816/1117);依从性、吸烟、高血压病、镇静镇痛药使用或依赖、麻醉方式、术式、手术时长、术灶位置、术毕至治疗的时间、治疗频度与单次时长、取穴为影响因素(P<0.05);其中,吸烟、高血压病、术灶与齿状线的关系、术毕至开始治疗的时间、单次治疗时长与频度、取穴为独立影响因素(P<0.05)。结论穴位治疗可有效缓解痔疮术后疼痛;应辨证论治、合理取穴、同穴异治;对于有影响因素或独立影响因素者宜采用早治疗、多穴位、强治法。

Objective To retrospectively analyze the efficacy and influencing factors of acupoint analgesic treatment after hemorrhoid surgery.Methods A total of 1117 cases with complete data on acupoint analgesic treatment after hemorrhoid surgery in People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine in the past 5 years were collected.The correlation between gender,age,compliance,smoking,diabetes,hypertension,use or dependence of sedative and analgesic drugs before operation,anesthesia method,operation type,operation duration,location and number of surgical foci,initial operation or not,time from operation to treatment,duration and frequency of treatment,number of points selected and other factors were analyzed.Results The total effective rate was 73.05%(816/1117).Compliance,smoking,hypertension,sedative and analgesic drug use or dependence,anesthesia mode,operation type,operation duration,surgical focus location,time from operation to treatment,treatment frequency and single duration were the influencing factors(P<0.05).Among them,smoking,hypertension,the relationship between surgical focus and dentate line,the time from the end of surgery to the start of treatment,the duration and frequency of single treatment,point selection were independent influencing factors(P<0.05).Conclusion Acupoint treatment can effectively relieve the postoperative pain of hemorrhoids.Treatment should be based on syndrome differentiation,reasonable selection of points,different treatment of the same points,for those who have influential factors or independent influential factors,early treatment,multiple acupoints and strong treatment should be adopted.
